血清MMP-11、MMP-26水平对结直肠癌诊断及预后评估的价值

The value of serum MMP-11 and MMP-26 levels in diagnosis and prognosis evaluation of colorectal cancer

目的 探讨结直肠癌患者血清基质金属蛋白酶-11(MMP-11)、基质金属蛋白酶-26(MMP-26)在结直肠癌诊断及预后评估的价值。方法 选取本院肿瘤科收治的68例结直肠癌患者进行作为研究对象(研究组),选取同期体检健康者68例作为对照组,采用酶联免疫吸附法(ELISA)检测所有受试者血清MMP-11、MMP-26水平,进一步分析二者表达与临床病理参数的关系及对结直肠癌诊断和预后价值。结果 研究组患者血清MMP-11、MMP-26表达水平均高于对照组(P<0.05);血清MMP-11、MMP-26表达均与分化程度、Dukes分期、淋巴结转移及远处转移相关(P<0.05);分别以血清MMP-11、MMP-26表达水平绘制受试者工作曲线(ROC)曲线显示,MMP-11的AUC为0.922,截断值>4.99(ng/ml),诊断敏感性为86.76%,特异性为82.35%,MMP-26的AUC为0.821,截断值>9.96(ng/ml),诊断敏感性为82.35%,特异性为80.88%;MMP-11、MMP-26高表达患者中位生存时间均明显低于低表达患者(P<0.05)。结论 MMP-11、MMP-26参与疾病进展,MMP-11、MMP-26高表达患者预后较差,可作为结直肠癌早期诊断及预后评估的潜在检测指标。

Objective To investigate the value of serum matrix metalloproteinase-11 (MMP-11) and matrix metalloproteinase-26 (MMP-26) in the diagnosis and prognosis of colorectal cancer (CRC). Methods Sixty-eight patients with CRC who were admitted to Department of Oncology of our hospital were selected as the study subjects (research group), and 68 healthy subjects who underwent physical examination were selected as the control group. Serum levels of MMP 11 and MMP 26 were detected by enzyme-linked immunosorbent assay (ELISA).The relationship between the expression of the two and clinicopathological parameters as well as their value in diagnosis and prognosis of CRC were further analyzed.Results The expression levels of serum MMP11 and MMP26 in research group were higher than those in the control group (P<0.05). The expressions of serum MMP11 and MMP26 were correlated with differentiation degree, Dukes stage, lymph node metastasis and distant metastasis (P<0.05). The receiver operating characteristic (ROC) curve of serum MMP11 and MMP26 expression levels showed that the AUC of MMP11 was 0.922 and the cutoff value was >4.99(ng/ml). The diagnostic sensitivity and specificity were 86.76%, and 82.35% respectively. The AUC of MMP26, the cutoff value,diagnostic sensitivity and specificity were 0.821, >9.96(ng/ml), 82.35%, and 80.88%, respectively. The median survival time of patients with high expression of MMP11 and MMP26 was significantly lower than that of patients with low expression (P<0.05).Conclusion MMP11 and MMP26 are involved in disease progression, and patients with high expression of MMP11 and MMP26 have poor prognosis, which can be used as potential detection indicators for early diagnosis and prognosis evaluation of CRC.
