中性粒细胞在医院感染性新生儿败血症早期诊断中的价值

The diagnostic value of detection of neutrophils in early diagnosis of nosocomial neonatal sepsis

目的探讨中性粒细胞在医院感染性新生儿败血症早期的临床诊断价值。方法对2012年6月至2017年5月收住廊坊市人民医院新生儿病房,确诊为医院感染性败血症的101例新生儿病例资料进行回顾性分析。对比发病前末次和发病后首次血常规结果中的中性粒细胞百分比、中性粒细胞计数变化,并通过绘制受试者工作特征曲线(ROC曲线)分析中性粒细胞百分比、计数对医院感染性败血症早期诊断的价值。结果医院感染性败血症早期中性粒细胞百分比、计数均高于发病前(P<0.05)。中性粒细胞百分比的最佳诊断截点为53.45%时,敏感度为76.2%,特异度为89.1%;中性粒细胞计数的最佳诊断截点为6.56×10^9/L,敏感度为59.4%,特异度为89.1%。结论中性粒细胞百分比在医院感染性败血症早期的诊断价值高;中性粒细胞计数对医院感染性败血症早期诊断具有一定的临床应用价值。

Objective To investigate the clinical value of detection of neutrophils in early diagnosis of nosocomial neonatal sepsis.Methods The clinical data about 101 neonates with nosocomial infectious sepsis who were treated in Langfang People’s Hospital from June 2012 to May 2017 were retrospectively analyzed.The changes of the percentage of neutrophils and neutrophil count in the last blood routine test results were compared before onset of sepsis and in the first blood routine test after onset.Moreover the receiver operating characteristic(ROC)curves were drawn to evaluate the clinical value of the two parameters in early diagnosis of nosocomial sepsis.Results The percentage of neutrophils and neutrophil count before onset of disease were signifcantly higher than those after onset of nosocomial sepsis(P<0.05).The optimal cut-off point of percentage of neutrophils in diagnosis of nosocomial sepsis was 53.45%,with sensitivity and specifcity being 76.2%and 89.1%,respectively.The optimal cut-off point of neutrophil count in diagnosis of nosocomial sepsis was 6.56×10^9/L,with the sensitivity and specificity being 59.4%and 89.1%,respectively.Conclusion The diagnostic accuracy of percentage of neutrophils in early diagnosis of nosocomial sepsis is higher,moreover,the neutrophil count has certain clinical application value in early diagnosis of nosocomial sepsis.
