某院静脉用药调配中心差错防范与管理

Prevention and Management of Error in the Center of Intravenous Drug Dispensing in the Hospital

目的控制和减少静脉用药调配中心(PIVAS)的差错事故,保证临床的安全用药。方法通过分析该院PIVAS的差错类型及发生差错的原因,制定出差错防范与管理措施。结果通过统计实施差错防范与管理措施前半年(2018年1—6月)及实施差错防范与管理措施后半年(2018年7—12月)静脉用药调配中心的发生的差错例数发现,各种类型的差错发生率大大减少,取得明显的成效。结论采取积极有效的差错防范与管理措施,可以有效控制和减少PIVAS的差错发生率,保证PIVAS所配置输液的质量与安全,使得临床用药的安全性及PIVAS的服务水平均有效提高。

Objective To control and reduce the error of the intravenous drug delivery center(PIVAS)to ensure the safe use of drugs.Methods By analyzing the types of errors in our hospital PIVAS and the causes of errors,we developed error prevention and management measures.Results The implementation of error prevention and management measures in the first half of the year(January-June 2018)and the implementation of error prevention and management measures in the second half of the year(July-December 2018),the number of errors in the occurrence of intravenous drug distribution centers found to be greatly reduced,and significant results have been achieved.Conclusion Adopting effective error prevention and management measures can effectively control and reduce the error rate of PIVAS,ensure the quality and safety of the infusions configured by PIVAS,and effectively improve the safety of clinical medications and the service level of PIVAS.
