球囊辅助跟踪技术在PCI术中桡动脉痉挛应用的有效性及安全性

Efficacy and safety of balloon assisted tracking technique guiding catheter through the spastic radial artery during percutaneous coronary intervention

摘要 目的:探讨经皮冠状动脉介入治疗中球囊辅助跟踪(BAT)技术导管通过桡动脉痉挛段的安全性和有效性。方法:回顾行冠状动脉介入治疗中桡动脉痉挛患者77例。采用BAT技术处理痉挛37例(BAT组),常规方法处理痉挛40例(常规处理组)。比较两种方法前臂血肿的发生率、手术时间、透视时间及更改手术途径的差异。结果:BAT组和常规组患者前臂血肿发生率分别为2.7%、15.0%,但差异无统计学意义(P>0.05)。BAT组37例导管均顺利通过桡动脉痉挛段血管,成功率达100%;而常规组有6例(15.0%)失败更改途径(P<0.05)。两组手术时间和透视时间比较无显著差异(P>0.05)。结论:经桡动脉径路PCI发生桡动脉痉挛时,采用BAT技术可有效、安全地使导管通过痉挛血管,优于常规处理技术。 Objective To evaluate the safety and effectiveness of balloon assisted tracking(BAT)technique dealing with radial artery spasm in percutaneous coronary intervention(PCI).Methods From December 2015 to December 2018,a total of 77 patients with radial artery spasm who received PCI through the radial artery approach were retrospectively analyzed.37 cases(BAT group)were treated with BAT technology,and 40 cases(conventional group)were treated with conventional method.The differences in incidence of forearm hematoma,operation time and fluoroscopy time were compared between the two groups.Results The incidence of forearm hematoma in BAT group and conventional group was 2.7%and 15.0%respectively,but the difference was not statistically significant(P>0.05).6 patients(15.0%)in the conventional group failed to passed through the spasmodic segment and converted to the contralateral radial artery approach or femoral artery approach(P<0.05).There was no significant difference between the two groups in operation time and fluoroscopy time(P>0.05).Conclusion BAT technique can effectively and safely guide the catheter through the spasmodic segment of radial artery during PCI through TRA,which is superior to conventional technique.
