超声引导下臂丛神经阻滞镇痛结合中医手法松解对肱骨外科颈骨折术后功能的影响

Effect of ultrasound-guided brachial plexus block analgesia combined with traditional Chinese medicine manual release on the postoperative function of humeral surgical neck fracture

目的:探讨超声引导下臂丛神经阻滞镇痛结合中医手法松解对肱骨外科颈骨折术后功能的影响。方法:回顾性分析我科收治肱骨外科颈骨折患者56例,随机分为实验组和对照组(n=28)。比较两组术后骨折愈合时间、VAS评分、Constant-Murley评分及不良反应。结果:实验组骨性愈合时间为11.2±2.4周,优于对照组(P=0.041)。两组患者术后V A S明显降低(P=0.007),但实验组术后3个月后V A S评分较对照组明显降低(P<0.05)。末次随访时,实验组Constant-Murley评分为85.8±11.6分,优于对照组(P=0.037)。实验组优良率为92.9%,高于对照组的71.4%(P=0.031)。结论:超声引导下臂丛神经阻滞镇痛结合中医手法松解可以较好的改善肱骨外科颈骨折术后功能的恢复,减少不良反应发生,值得临床推广。

Objective To investigate the effect of ultrasound-guided brachial plexus block analgesia combined with traditional Chinese medicine manual release on the postoperative function of humeral surgical neck fracture.Methods 56 cases of humeral surgical neck fracture in our department were retrospectively analyzed.They were randomly divided into experimental group and control group(n=28).The fracture healing time,VAS score,Constant-Murley score and adverse reactions were compared.Results The bone healing time of the experimental group was less,11.2+2.4 weeks(P=0.041).VAS decreased significantly after operation(P=0.007),but the VAS score in the experimental group was lower at 3 months after operation(P<0.05).At latest follow-up,the Constant-Murley score of the experimental group was 85.8+11.6,which was better than that of the control group(P=0.037).The experimental group had higher excellent and good rate(92.9%VS 71.4%,P=0.031).Conclusion Ultrasound-guided brachial plexus block analgesia combined with traditional Chinese medicine manual release can improve the functional recovery of humeral surgical neck fracture and reduce the occurrence of adverse reactions,which is worthy of clinical promotion.
