无创呼吸机在ICU急性心力衰竭合并呼吸衰竭治疗中的应用效果探讨

Application of Non-invasive Ventilator in the Treatment of Acute Heart Failure Combined with Respiratory Failure in ICU

目的分析ICU急性心力衰竭合并呼吸衰竭治疗中无创呼吸机的应用效果。方法选取该院ICU2015年1月—2019年2月收治的72例急性心力衰竭合并呼吸衰竭患者为研究对象,随机分为两组,给予对照组患者持续低流量吸氧治疗,给予观察组患者无创呼吸机辅助治疗,对比两组治疗前后cTnI(心肌肌钙蛋白I)、NT-pro BNP(血清氨基末端B型钠尿肽原)水平变化情况、血气指标变化情况。结果观察组治疗12 h时的cTnI、NT-pro BNP水平分别为(5.12±0.57)ng/mL、(645.85±112.69)pg/mL,均低于对照组,观察组治疗24 h时的cTnI、NT-pro BNP水平分别为(4.62±1.02)ng/mL、(511.25±105.69)pg/mL,同样均低于对照组差异有统计学意义(t=20.188、15.632、15.632、22.025,P<0.05);观察组治疗后PaCO2为(6.02±0.45)mmHg,低于对照组,pH、SaO2、PaO2分别为(7.41±0.22)、(94.58±5.68)%、(7.99±2.58)mmHg,均高于对照组差异有统计学意义(P<0.05)。结论ICU急性心力衰竭合并呼吸衰竭治疗中无创呼吸机的应用效果显著。

Objective To analyze the application effect of non-invasive ventilator in the treatment of acute heart failure and respiratory failure in ICU.Methods 72 patients with acute heart failure and respiratory failure admitted to the hospital from January 2015 to February 2019 were randomLy divided into two groups.The patients in the control group were given low-flow oxygen therapy and the patients in the observation group were given non-invasive ventilator assisted treatment,compared with the changes of cTnI(cardiac troponin I),NT-pro BNP(serum amino terminal B-type natriuretic peptide)and blood gas index before and after treatment.Results The levels of cTnI and NT-pro BNPat12 h in the observation group were(5.12±0.57)ng/mL and(645.85±112.69)pg/mL,respectively,which were lower than those of the control group.The observation group received cTnI and NT-pro BNP levels at 24 h were(4.62±1.02)ng/mL and(511.25±105.69)pg/mL,which were also lower than the control group,the difference was statistically significant(t=20.188,15.632,15.632,22.025,P<0.05).The observation group was treated with PaCO2(6.02±0.45)mmHg,lower than the control group,pH,SaO2,PaO2 were(7.41±0.22),(94.58±5.68)%,and(7.99±2.58)mmHg,respectively,which were higher than the control group,the difference was statistically significant(P<0.05).Conclusion The application of noninvasive ventilator in the treatment of acute heart failure and respiratory failure in ICU is significant.
