Analysis of Our Hospital Easily Confused Drugs and Coping Strategies

目的保障医院门诊易混淆药品的用药安全,提高药师对易混淆的药品的重视,防止调配差错事故,提高医院药事管理水平。方法将医院2018门诊西药和中成药中所有易混淆的药品进行归纳总结,分析并总结易混淆药品调配差错发生的原因,为规避差错给出合理建议。结果经分类整理得出10组易混淆药品,其中外包装相似的药品3组,名称相似的药品2组,通用名相同生产厂家不同的药品3组,通用名相同剂型不同的药品2组。将医院易混淆的药品总结并建立文本。结论在工作中药学人员务必重视易混淆药品,并采取相应防范措施,减少处方中易混淆药品调配差错的发生,保障患者用药安全,做好居民健康的守门人。

Objective To ensure the drug safety of confusable drugs in outpatient department of our hospital, improve pharmacists’ attention to confusable drugs, prevent deployment errors and accidents, and improve our hospital’s drug management level. Methods All the confusable drugs in the 2018 outpatient department of our hospital were summarized, and the causes of the mixing errors of confusable drugs were analyzed and summarized, so as to provide reasonable suggestions for avoiding errors. Results 10 groups of confusable drugs were classified and sorted out, including 3 groups of drugs with similar packaging, 2 groups of drugs with similar names, 3 groups of drugs with the same generic name and different manufacturers, and 2 groups of drugs with the same generic name and different dosage forms. Summarize the confusing drugs in our hospital and establish the text. Conclusion Pharmacists must pay attention to confusable drugs in the work, and take corresponding preventive measures, reduce the occurrence of dispensing errors of confusable drugs in the prescription, ensure the safety of patients’ medication, and do a good job in the gatekeeper of residents’ health.
