以360度评估为主的综合评估是保证住院医师规范化培训质量的关键 被引量:24

A comprehensive assessment based on 360-Degree Evaluation is the key to ensuring the quality of residency training

摘要 目的本研究旨在探索以360度评估为主的综合评估在住院医师规范化培训中的应用价值,为提高住院医师规范化培训质量、推广有效的临床教学评估手段提供依据。方法运用中国医师协会360度评估系统对武汉普爱中美全科项目住院医师、临床师资进行评估,收集分析了自2017年以来共计20个月的数据资料。结果360度评估耗时短,超过87%的带教医师在10 min内即可完成一份评估问卷,接受度高;非医学知识类临床胜任力的评估在全部评分中的比重占近2/3且对医学知识的评价与执业医师考试结果显示出正相关;根据住院医师的反馈,带教医师教学能力的10个指标上普遍有所提高。结论360度评估可操作性好,实现了对多方面核心胜任力的全方位覆盖,体现了系统的“保底”而非“拔优”的特点和纠偏功能,对师资带教水平及培训基地教学质量能够起到监督和促进作用。以360度评估为主的综合评估体系对提高和保障住培质量有重要意义。 Objective The purpose of this study is to explore the application value of the 360-Degree Evaluation of the Chinese Medical Doctor Association(CMDA)in residency training and to provide evidence for improving the quality of discipline training and promoting effective clinical teaching evaluation methods.Methods Using the 360-Degree Evaluation System of the CMDA to evaluate the residents and teaching attendings in Sino-US Pilot General Practice Standardized Residency Training Project atWuHan.Collected and analyzed data in a total of 20 months since 2017.Results Completing the 360-Degree Evaluation takes a short time,more than 87%of the teaching attendings can complete an assessment questionnaire within 10 minutes,makes it highly accepted.The proportion of questions in the 360-Degree Evaluation for core competencies except“medical knowledge”accounts for nearly two-thirds of all assessments.Its evaluation of“medical knowledge”is positively correlated with the results of practitioner exam.According to feedback from residency trainees,the teaching ability of attendings has generally improved in 10 indicators.Conclusion The 360-Degree Evaluation has good operability,and achieves full coverage of all core competencies,reflecting its feature of“Guarantee basic and acceptable training standards”rather than“selecting excellence”and function of“correction”.At the same time,it can supervise and improve the teaching skill of the teaching attendings and the teaching quality of the residency program.A comprehensive assessment system based on a 360-Degree Evaluation is critical to improving and guaranteeing the quality of residency training.
