医护一体化分层级责任制护理对前列腺增生症患者的影响

目的探讨医护一体化分层级责任制护理对前列腺增生症患者的影响。方法选择2017年1月至2019年1月入住本院、需要实施经尿道手术治疗的前列腺增生症患者92例,随机分为对照组与研究组。对照组采用常规责任制护理,研究组采用医护一体化分层级责任制护理。对比两组炎性反应因子水平、生活质量以及术后并发症发生情况。结果干预前,两组患者炎性反应因子水平差异无统计学意义(均P>0.05);干预后,研究组CRP、TNF-α、IL-6水平分别为(71.20±8.27)mg/L、(25.74±4.33)ng/L、(30.87±4.96)ng/L,对照组分别为(95.14±9.65)mg/L、(36.41±7.58)ng/L、(43.28±6.24)ng/L,研究组各指标状况显著优于对照组(均P<0.05)。干预前,两组生活质量指标评分差异无统计学意义(均P>0.05);干预后,研究组社会功能、躯体功能、角色功能、认知功能等生活质量指标评分分别为(72.6±2.5)分、(75.4±3.1)分、(76.2±3.1)分、(73.2±5.5)分,对照组分别为(58.8±4.9)分、(63.4±3.6)分、(66.7±2.8)分、(53.4±4.1)分,研究组显著高于对照组(均P<0.05)。研究组尿道口狭窄、膀胱颈挛缩、血尿等术后并发症发生率为10.9%,对照组为23.92%(P<0.05)。结论在经尿道手术治疗前列腺增生症中应用医护一体化分层级责任制护理,能够提升前列腺增生症患者的生活质量水平,减少并发症的发生,同时还可降低炎症因子水平,效果理想,值得推广。

Objective To explore the effect of doctor-integrated-with-nurse hierarchical responsibility nursing system for patients with benign prostatic hyperplasia.Methods A total of 92 patients who underwent transurethral resection of benign prostatic hyperplasia were enrolled in our hospital from January,2017 to January,2019,and were randomly divided into a control group and a study group.The control group were cared in terms of the routine responsibility system,and the study group in terms of doctor-integrated-with-nurse hierarchical responsibility nursing system.The levels of inflammatory response factors,quality of life,and postoperative complications were compared between the two groups.Results There were no statistical differences in the levels of inflammatory response factors before the intervention between the two groups(all P>0.05);after the intervention,the levels of CRP,TNF-α,and IL-6 were(71.20±8.27)mg/L,(25.74±4.33)ng/L,and(30.87±4.96)ng/L in the study group,and were(95.14±9.65)mg/L,(36.41±7.58)ng/L,and(43.28±6.24)ng/L in the control group(all P<0.05).There were no statistical differences of the scores of quality of life between these two groups before the intervention;and after the intervention,the scores of social function,physical function,role function,and cognition were(72.6±2.5),(75.4±3.1),(76.2±3.1),and(73.2±5.5)in the study group,and were(58.8±4.9),(63.4±3.6),(66.7±2.8),and(53.4±4.1)in the control group(all P<0.05).The incidence of postoperative complications,such as urethral stricture,bladder neck contracture,and hematuria,in the study group was 10.9%,and that in the control group was 23.92%(P<0.05).Conclusion In the treatment of benign prostatic hyperplasia by transurethral surgery,the application of doctor-integrated-with-nurse hierarchical responsibility nursing system can improve the patients’quality of life,reduce the incidence of complications and the levels of inflammatory factors,so it is worth for promotion.
