摘要
目的:分析男性乳腺癌患者腋窝淋巴结转移的相关因素。方法:提取SEER数据库2010年至2014年病理诊断为浸润性乳腺癌的男性病例资料1636例,回顾性分析临床病理特征与腋窝淋巴结状态的相关性。结果:1636例患者中,739例(45.17%)腋窝淋巴结转移。经单因素分析,淋巴结状态与肿瘤位置(P=0.001)、肿瘤组织学分级(P<0.001)、肿瘤大小(P<0.001)及人表皮生长因子受体2(P=0.015)存在相关性,与年龄(P=0.295)、雌激素受体(P=0.725)、孕激素受体(P=0.848)无相关性。二分类多因素Logistic回归分析显示,肿瘤位置(OR=1.368,95%CI 1.083~1.723,P=0.009)、肿瘤组织学分级(OR=1.241,95%CI 1.029~1.497,P=0.024)和肿瘤大小(OR=1.041,95%CI 1.029~1.052,P<0.001)是淋巴结状态的独立预测因素,人表皮生长因子受体2不是独立预测因素(OR=1.150,95%CI 0.800~1.653,P=0.449)。结论:肿瘤的位置、大小及组织学分级是男性乳腺癌腋窝淋巴结状态的独立预测因素。
Objective:To analyze the related factors of axillary lymph node(ALN)metastases in male patients with breast cancer.Methods:1636 patients with male breast cancer(MBC)were extracted from the Surveillance,Epidemiology,and End Results(SEER)database diagnosed between 2010 and 2014 years.The association between ALN status and clinicopathological features were retrospectively analysed.Results:The ALN metastatic rate of all enrolled population was 45.17%.The ALN status was significantly associated with the tumor site(P=0.001),grade(P<0.001),tumor size(P<0.001)and human epidermal growth factor-2(HER-2)(P=0.015)in the univariate analysis.But there was no significant difference with the age(P=0.295),estrogen receptor(P=0.725)and progestogen receptor(P=0.848).In binary logistic analysis,tumor site(OR=1.368,95%CI 1.083~1.723,P=0.009),grade(OR=1.241,95%CI 1.029~1.497,P=0.024)and tumor size(OR=1.041,95%CI 1.029~1.052,P<0.001)were independent predictors for ALN metastasis in MBC.However,HER-2 status(OR=1.150,95%CI 0.800~1.653,P=0.449)was not the independent predictor for ALN status.Conclusion:Tumor size,tumor site and histological grade were the independent predictors for ALN metastasis in MBC.
