摘要
目的探讨经导管肝动脉化疗栓塞术(TACE)联合经皮穿刺微波消融术治疗肝癌的疗效及对血清甲胎蛋白(AFP)、癌胚抗原(CEA)、糖类抗原19-9(CA19-9)水平的影响。方法按照治疗方法不同将76例肝癌患者分为对照组和观察组,每组38例。对照组患者接受TACE治疗,观察组患者接受TACE联合经皮穿刺微波消融术治疗。比较两组患者的临床疗效、2年生存率及治疗前后血清AFP、CEA、CA19-9水平。结果观察组患者的总有效率与对照组比较,差异无统计学意义(P﹥0.05);观察组患者的2年生存率为56.2%,明显高于对照组的23.6%(P﹤0.01);治疗后,观察组和对照组患者的血清CA19-9、AFP、CEA水平均明显低于本组治疗前,且观察组患者的血清CA19-9、AFP、CEA水平均明显低于对照组(P﹤0.01)。结论TACE联合经皮穿刺微波消融术治疗肝癌的远期疗效较好,且可以有效降低患者的血清肿瘤标志物水平。
Objective To investigate the therapeutic effect of transcatheter arterial chemoembolization(TACE)combined with percutaneous microwave ablation on liver cancer and its influence on serum levels of alpha-fetoprotein(AFP),carcinoembryonic antigen(CEA)and carbohydrate antigen 19-9(CA19-9).Method A total of 76 patients with liver cancer were divided into control group and study group according to different treatment methods,including 38 cases in each group.Patients in control group received TACE,while those in study group were given TACE combined with percutaneous microwave ablation.The clinical efficacy,2-year survival rate and serum AFP,CEA and CA19-9 levels before and after treatment were compared between the two groups.Result There was no significant difference in the complete response rate between study group and control group(P>0.05);the 2-year survival rate of patients in study group was 56.2%,which was significantly higher than the 23.6%in control group(P<0.01);after treatment,the levels of serum CA19-9,AFP and CEA in both groups were significantly decreased as compared with those before treatment,and the levels of serum CA19-9,AFP and CEA in study group were significantly lower than those in the control group(P<0.01).Conclusion TACE combined with percutaneous microwave ablation for liver cancer demonstrates fairly good long-term effect and can effectively reduce the serum AFP,CEA and CA19-9 levels of patients.
