老年人自主神经功能障碍的患病率及其影响因素研究

Prevalence of autonomic dysfunction and its influencing factors in Chinese elderly

目的调查天津市蓟州社区老年人自主神经功能障碍的患病率及影响因素。方法采用横断面研究方法,对老年人进行问卷调查,调查自主神经功能障碍的患病率并分析其影响因素。结果最终纳入1292例老年人中,有196例患有自主神经功能障碍,患病率为15.2%(196/1292)。自主神经功能障碍症状以尿频/尿急/尿不尽(19.7%,255/1292)和便秘(15.9%,205/1292)为主。多因素Logistic回归分析结果显示,女性(OR=1.808,95%CI:1.253~2.607)、年龄75~85岁(OR=1.573,95%CI:1.109~2.232)、全麻手术史(OR=1.552,95%CI:1.044~2.307)、睡眠障碍(OR=2.906,95%CI:1.506~2.916)、糖尿病(OR=1.791,95%CI:1.197~2.678)、头痛(OR=2.589,95%CI:1.482~4.520)是发生自主神经功能障碍的危险因素。结论天津市蓟州社区老年人自主神经功能障碍的患病率较高,应积极评估老年患者潜在的危险因素,尽早识别和治疗自主神经功能障碍症状。

Objective To investigate the prevalence of autonomic dysfunction and its influencing factors in the elderly in Jizhou community of Tianjin.Methods By using a cross-sectional study,a questionnaire survey was conducted in the elderly in order to investigate the prevalence of autonomic dysfunction and its influencing factors.Results A total of 1292 elderly patients were enrolled.Of them,196 cases had autonomic dysfunction(15.2%,196/1292).The main symptoms of autonomic dysfunction were frequent urination,urination urgency,urination incontinence(19.7%,255/1292)and constipation(15.9%,205/1292).Multivariate Logistic regression analysis showed that women(OR=1.808,95%CI:1.253~2.607),75-85 years of age(OR=1.573,95%CI:1.109~2.232),general anesthesia history(OR=1.552,95%CI:1.044~2.307),sleep disorders(OR=2.906,95%CI:1.506~2.916),diabetes(OR=1.791,95%CI:1.197~2.678)and headache(OR=2.589,95%CI:1.482~4.520)were risk factors for autonomic dysfunction.Conclusions The prevalence of autonomic dysfunction is high in the elderly in Jizhou community of Tianjin city.It is necessary to pay great attention to potential risk factors of autonomic dysfunction in the elderly.And the autonomic dysfunction symptoms should be diagnosed and treated as early as possible.
