小儿肺炎和反应性血小板增多症之间的关系

Pneumonia and reactive thrombocytosis in children

目的探讨小儿肺炎和反应性血小板增多之间的关系。方法回顾性分析2016年3月至2018年6月期间杭州市第一人民医院肺炎支原体患儿的临床资料,采用全自动血细胞分析仪测定200例肺炎患儿和45例正常小儿的血小板参数和白细胞计数(WBC)。按照有无血小板增多症(RT)将肺炎患儿分为RT组(38例)和非RT组(162例)。结果 200例肺炎支原体患儿合并RT 38例(19.0%),血小板减少症1例(0.5%)。RT组肺外并发症发生率(28.95%)高于非RT组(14.20%),差异有统计学意义(χ~2值为4.745,P<0.05)。三组血小板计数(PLT)、血小板体积(MPV)、血小板分布宽度(PDW)及WBC比较差别均有统计学意义(F值为94.184、23.527、28.317、7.380,均P<0.01)。两两比较后,RT组PLT值高于非RT组和对照组,RT组和非RT组的MPV、PDW、WBC均高于对照组,RT组MPV、PDW均低于非RT组。结论小儿肺炎和反应性血小板增多症之间有关联,肺炎支原体患儿合并RT发生率高,合并RT患儿肺外并发症发生率高。

Objective To investigate the relationship between pneumonia and reactive thrombocytosis in children.Methods A retrospective analysis of clinical data of children with mycoplasma pneumoniae pneumonia admitted to our hospital during the period from March 2016 to June 2018 was conducted,using automatic blood cell analyzer to determine platelet parameters and white blood cells(WBC) in 200 children with pneumonia and 45 normal children.The children were divided into Reactive thrombocytosis(RT) group(38 cases) and non-RT group(162 cases) according to the presence of thrombocytopenia(RT).Results Of the 200 children with mycoplasma pneumoniae,38(19.0%) had RT and 1(0.5%) had thrombocytopenia.The incidence of extrapulmonary complications in the RT group(28.95%) was higher than that in the non-RT group(14.20%)(χ~2=4.745,P<0.05).There were significant differences in platelet count(PLT),platelet volume(MPV),platelet distribution width(PDW) and WBC among the three groups(F=94.184,23.527,28.317 and 7.380,respectively,P<0.01 for all).The PLT value of RT group was higher than that of non-RT group and the control group.MPV,PDW and WBC of RT group and non-RT group were higher than those of the control group.MPV and PDW of the RT group were lower than those of non-RT group.Conclusion There is a relationship between infant pneumonia and reactive thrombocytopenia.The incidence of RT in children with mycoplasma pneumoniae is high.The incidence of extrapulmonary complications is high in children with RT.
