标本同治治疗麦粒肿

The treatment for hordeolum with treating symptom and root cause simultaneously

目的讨论麦粒肿的中医治法。方法麦粒肿属本虚标实,本虚为正气不足,标实乃脾胃蕴热;就主证而言:眼睑红肿热痛为标,肝脾蕴热为本。肝脾蕴热,肝主疏泄而藏血,脾主运化而统血,肝有热则伤肝,肝伤则气机郁滞而血失调畅,脾伤则升降紊乱而血失统摄,故气滞、血瘀应责之肝脾;又因脾属土而能制水,为津液化生转输之枢,脾虚则不能散精,脾主肌肉,"肉轮"属脾,精不能营养"肉轮"。肝调畅气机失常,不能维持脾胃升降运化,而使气血凝滞,上壅于眼睑而发病。结果谢氏经验方治疗麦粒肿仅治其标,未治其本,疗效易反复。本文从标本同治着手,认为麦粒肿发病的病机应责之于肝脾,并按成脓与否将其分为三期:初期(风热外感),中期(热毒上攻),后期(热毒炽盛而正气未衰型、脾胃伏热型、正虚毒恋型)。结论不同时期不同证型需要加用内服不同的汤液,从而达到标本同治,最终实现治病求本的目的。

Objective To explore the TCM treatment of hordeolum.Methods The hordeolum belongs to the asthenia in origin of asthenia of healthy qi,and sthenia in superficialit y of spleen stomach heat accumulation.As far as the main syndrome is concerned:eyelid redness,swelling,heat and pain are the signs,while liver and spleen gather heat is the basis.The liver and spleen accumulate heat,the liver mainly dredge and accumulate blood,the spleen mainly transport and regulate the blood,the liver has heat,the liver injury,the liver injury qi stagnation and blood imbalance,the spleen injury rise and fall disorder and blood loss,so qi stagnation,blood phlegm should blame the liver and spleen.Also because the spleen belongs to the soil and can control water,for the body fluid transformation pivot.Spleen deficiency can not scatter essence,spleen master muscle,"meat wheel"belongs to the spleen,essence can not nutrition"meat wheel",liver regulating qi activity is abnormal,which cannot maintain the spleen operation,and make the blood stagnate and indicates on the eyelids and the onset.Results Xie’s experience for the treatment of hordeolum only treat its superficiality,instead of its origin,the curative effect is easy to repeat.This article starts from the treating symptom and root cause simultaneously,and the disease should be responsible to the liver and spleen,and divided into three stages according to whether it is pyogenic or not:initial stage(wind hot exogenous),middle stage(hot poison attack),late stage(toxic heat flourishing and healthy vital qi type,spleen and stomach thermal type,and lingering of pathogenic factors due to asthenia of healthy qi).Conclusion Different syndrome types in different periods need to be added with different decoction,so as to achieve the treating symptom and root cause simultaneously,and ultimately achieve the purpose of treating diseases and seeking the root.
