直接前方入路微创全髋关节置换术研究进展

Research advances in minimally invasive total hip arthroplasty through direct anterior approach

在各种微创技术的快速发展下,多种疾病可应用微创手术进行治疗,极大地降低了患者手术治疗的创伤性,同时也有助于实现术后快速康复。直接前方入路(DAA)微创全髋关节置换术(THA)是一种微创治疗手段,具有不需要切断肌肉组织,对患者机体损伤较小,且微创手术切口较小,可有效控制术后切口感染、术后关节脱位等并发症的发生率,有助于实现术后快速康复等优点,在骨科临床应用具有明显优势。现阶段临床中对于DAA微创THA的推广时间还相对较短,部分临床医生对于此种治疗手段的认知还不够全面。本文对DAA微创THA的适应范围、手术方法等多个方面进行综述,为临床应用提供参考。

With the rapid development of various minimally invasive techniques, a variety of diseases can be treated by minimally invasive surgery, which greatly reduces the trauma of patients’ surgical treatment and helps achieve rapid postoperative recovery. Direct anterior approach(DAA) minimally invasive total hip arthroplasty(THA) is a minimally invasive treatment method, which don’t need to cut off the muscle tissue, with less damage to patients’ bodies and smaller incision. It can effectively control the incidence of postoperative complications such as postoperative incision infection and joint dislocation, which helps to achieve rapid postoperative recovery, and has significant advantages in orthopaedic clinical application. At present, the promotion of DAA minimally invasive THA is still relatively short in clinical practice, and some clinicians’ cognition of this treatment method is not comprehensive enough. In this paper, the application scope and operation methods of DAA minimally invasive THA are reviewed, providing reference for clinical application.
