兰州市1498名居民骨密度调查结果分析 被引量:2

Analysis of bone mineral density of 1498 residents in Lanzhou

摘要 目的调查兰州市1 498名居民骨密度情况及相关影响因素。方法在兰州市多阶段分层整群随机抽样调查1 498名居民,采用双能X线骨密度仪测定左前臂和右足跟骨密度,对骨密度变化进行t检验。将骨密度与居民年龄、体质指数、血压、血清总胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白、血尿酸、糖化血红蛋白、维生素D、血钙、血磷进行Pearson相关性分析,再将相关性因素进行多元逐步回归分析。结果 1 498名居民左前臂骨量减少占45.79%,骨质疏松占17.96%,右足跟骨量减少占37.05%,骨质疏松占17.89%。左前臂骨质疏松组的年龄、血清总胆固醇、低密度脂蛋白、糖化血红蛋白均高于骨量减少组(P<0.05);右足跟骨质疏松组年龄、低密度脂蛋白均高于骨量减少组(P<0.05)。右足跟骨质疏松组体质指数较骨量减少组低(P<0.05);左前臂和右足跟骨密度均与体质指数、收缩压、舒张压、甘油三酯、血尿酸、维生素D、血钙呈正相关。右足跟骨密度还与糖化血红蛋白呈正相关。左前臂和右足跟骨密度均与年龄、高密度脂蛋白、血磷呈负相关。结论骨代谢的影响因素复杂,左前臂较右足跟易发生骨代谢异常;左前臂骨代谢异常男性>女性,右跟骨骨代谢异常女性>男性。针对这些影响因素采取应对措施有助于防治骨代谢异常。 Objective To investigate the incidence of abnormal bone metabolism in Lanzhou city residents.Methods 1 498 residents,aged 18-86,were randomly selected from the multi-stage stratified cluster survey in Lanzhou.The bone density of the left forearm and right heel was measured by dual-energy X-ray absorptionmetry,and the changes of bone density were tested by t-test.Bone mineral density and residents of the age,body mass index,blood pressure,serum total cholesterol,triglycerides,high-density lipoprotein cholesterol(hdl-c),low density lipoprotein cholesterol(hdl-c),uric acid,glycosylated hemoglobin,blood,blood calcium,vitamin D blood phosphorus Pearson correlation analysis,multiple stepwise regression analysis and correlation factors,it is concluded that the main risk factors.Results The decrease of left forearm bone mass was 45.79% and osteoporosis 17.96% in 1 498 cases.Bone loss in the right heel accounted for 37.05% and osteoporosis 17.89%.The age,serum total cholesterol,low-density lipoprotein and glycosylated hemoglobin in the left forearm osteoporosis group were all higher than those in the bone mass reduction group(P<0.05).Age and LDL in the right heel osteoporosis group were higher than those in the bone mass reduction group(P<0.05).The BMI of the right heel osteoporosis group was lower than that of the bone mass reduction group(P<0.05).BMD of the left forearm and right heel was positively correlated with BMI,systolic blood pressure,diastolic blood pressure,triglyceride,serum uric acid,vitamin D and blood calcium.Bone mineral density of right heel was positively correlated with hemoglobin a1 c.The BMD of left forearm and right heel was negatively correlated with age,HDL and blood phosphorus.Conclusion The factors affecting bone metabolism are complex.The left forearm is more prone to abnormal bone metabolism than the right heel.Men are more likely than women to have abnormal bone metabolism in the left forearm,while women are more likely than men to have abnormal bone metabolism in the right calcaneus.It is helpful to take measures to prevent and cure abnormal bone metabolism.
