苔藓样药疹的皮肤镜学特征

Dermoscopic features of lichenoid drug eruption

目的分析苔藓样药疹(lichenoid drug eruption,LDE)的皮肤镜学特征。方法回顾性分析8例LDE患者临床资料和皮肤镜表现。结果8例患者160个LDE皮损中,肉眼下可观察到皮损有Wickham纹13个(8.13%)、色素沉着84个(52.50%)、白色鳞屑87个(54.38%)、红色背景101个(63.13%),未见血管改变;皮肤镜下可观察到皮损有Wickham纹80个(50.00%),色素沉着114个(71.25%)、血管改变76个(47.50%)、白色鳞屑95个(59.38%)和红色背景137个(85.63%);皮肤镜下Wickham纹、色素沉着、血管改变、白色鳞屑和红色背景的检出率明显高于肉眼下(P<0.05)。结论LDE的皮肤镜学表现包括Wickham纹、色素沉着、血管改变、白色鳞屑和红色背景,与扁平苔藓类似,皮肤镜有助于LDE诊断和鉴别诊断。

Objective To analyze the dermoscopic features of lichenoid drug eruption(LDE).Methods The clinical data and dermoscopic manifestations of 8LDE patients(160lesions)were retrospectively analyzed.Results Of these 160LDE lesions,there were 13lesions of Wickham striae(8.13%),84of pigmentation(52.50%),87of white scales(54.38%),101of red background(63.13%)and no blood vessels under the naked eye,while there were 80lesions of Wickham striae(50.00%),114of pigmentation(71.25%),76of blood vessel changes(47.50%),95of white scales(59.38%)and 137of red background(85.63%)under dermoscope.The detection rates of Wickham striae,pigmentation,blood vessel changes,white scales and red background under dermoscope were higher than those by the naked eyes(P<0.05).Conclusion The dermoscopic manifestations of LDE include Wickham striae,pigmentation blood vessel changes,white scales and red background,which is similar or identical to those of lichen planus.Dermoscopy contributes to the diagnosis of LDE and differential diagnosis.
