血清铁测定试剂对C反应蛋白测定的干扰价值研究

摘要 目的探讨血清铁测定试剂对C反应蛋白(C-reactive protein,CRP)测定的干扰机理及价值。方法收集2017年2月至2018年2月,山东大学齐鲁医院沂南分院沂南县人民医院进行血液检查的患者164例,收集患者血清,采用不同的方法对患者血清中的血清铁、CRP、白细胞介素-6(interleukin-6,IL-6),白细胞介素-8(interleukin-8,IL-8),IgA、IgG、IgM水平进行检测。结果与对照组检测方法比较,研究组1中hs-CRP水平的差异有统计学意义(t=1.976,P<0.05)。研究组2中血清铁水平低于对照组,差异有统计学意义(t=1.654,P<0.05),hs-CRP水平差异有统计学意义(t=21.937,P<0.05)。研究组3血清铁水平差异有统计学意义(t=21.647,P<0.05)。结论应用全自动生化分析仪器对患者的血清样本进行分析,应当注意指标检测的相互顺序,尽可能避免血清铁检测试剂对CRP水平的影响。 Objective To discuss the interferon effect of serum iron assay reagent on the determination of C-reactive protein(CRP).Methods 164serum samples were collected from patients in the People′s Hospital of Yinan County during February 2017and February 2018and serum levels of iron,CRP,IL-6,IL-8,IgA,IgG and IgM were measured in parallel using 4methods as control group(measured using standard method),studyⅠ(measuring iron first,then others),studyⅡ(replacing iron reagentⅠwith non-enzymatic water)and studyⅢ(replacing iron reagentⅡwith non-enzymatic water).Results Compared to control group,there were significant differences in hs-CRP levels in study groupⅠ(t=1.976,P<0.05)and study groupⅡ(t=21.937,P<0.05),not in study groupⅢ.Meanwhile,compared to control group,the differences in iron levels in study groupⅡ(t=1.654,P<0.05)and study groupⅢ(t=21.647,P<0.05)were also statistically significant.However,there were no differences in other biomarkers.Conclusions The serum biomarker detection order and the serum iron assay reagents may affect the serum detection levels of hs-CRP,especially the reagentⅠ.Therefore,when using the automatic biochemical analyzer to measure the serum levels of hs-CRP,the above influencing factors should be considered.
