瓜蒌牛蒡汤合阳和汤与中药外敷联合应用对哺乳期急性乳腺炎早期患者炎症介质水平和中医证候积分的影响

Effect of Gualou Niubao Decoction and Yanghe Decoction Combined with External Application of Chinese Medicine on IL-6 and CRP Levels in Early Patients with Acute Breast-Feeding Mastitis

目的探讨瓜蒌牛蒡汤合阳和汤联合中药外敷治疗哺乳期急性乳腺炎早期患者的临床效果。方法根据治疗方法不同,将80例哺乳期急性乳腺炎早期患者分为试验组与对照组,各40例。对照组给予静脉注射青霉素联合中药外敷治疗,试验组采用瓜蒌牛蒡汤合阳和汤联合中药外敷治疗,比较两组患者临床疗效、临床症状改善时间、不良反应发生情况以及治疗前后相关炎症介质水平、中医证候积分。结果试验组患者治疗有效率为95.00%,明显高于对照组的80.00%,差异有统计学意义(P<0.05);试验组患者乳房肿痛消失时间、体温恢复时间、肿块消失时间以及通乳时间均优于对照组,差异有统计学意义(P<0.05);治疗后,试验组患者炎症介质指标以及中医证候积分优于对照组,差异有统计学意义(P<0.05);两组患者治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论瓜蒌牛蒡汤合阳和汤联合中药外敷治疗哺乳期早期急性乳腺炎临床效果优于单一用药,可有效改善患者临床症状,减轻炎症反应,且安全性较高。

Objective To explore the clinical effects of Gualou Niubao Decoction and Yanghe Decoction, combined with external application of Chinese medicine on patients in early acute mastitis in lactation period. Methods According to the different treatment methods, 80 cases of early breast-feeding patients with acute mastitis were divided into experimental group and control group, with 40 cases in each group. The control group was treated with intravenous penicillin combined with external application of Chinese medicine, while the experimental group was treated with Gualou Niubao Decoction and Yanghe Decoction combined with external application of Chinese medicine. The clinical efficacy, improvement time of clinical symptoms, occurrence of adverse reactions, the level of related inflammatory mediators before and after treatment, and scores of TCM syndromes were compared between the 2 groups. Results Theeffective rate of treatment of patients in the experiment group was 80.00%(32/40), which was lower than that of the experimental group(95.00%(38/40),(P<0.05);in the experimental group, the disappearance time of swelling and pain, the recovery time of body temperature, the disappearance time of mass and the time of lactation were better than those of the control group(P<0.05). After treatment, the inflammation index and TCM syndrome score of the experimental group were better than the control group,(P<0.05). There was no significant difference in the adverse reactions between the 2 groups(P>0.05). Conclusion The clinical effects of Gualou Niubao Decoction and Yanghe Decoction combined with external application of Chinese medicine in the treatment of acute mastitis in early lactation period is better than that of single drug, which can effectively improve the clinical symptoms of patients, alleviate inflammation and has higher safety.
