门诊药房中药调配差错原因分析与防范措施

Cause analysis and prevention of misallocation of traditional Chinese medicine in outpatient pharmacy

目的分析门诊药房中药调配差错原因,并总结防范对策。方法选择2016年6月-2018年12月医院中药处方调配中发生差错的50例作为研究对象,对50例患者的中药调剂情况进行分析,分析影响中药调剂质量的原因。结果50例中药调剂差错病例中20例受到处方因素影响而出现差错,占40.00%,处方录入有误10例,审方差错6例,忽视处方脚注4例;受调配因素影响而出现差错16例,占比32.00%,调剂量误差10例,饮片调剂过程浪费4例,药品遗漏2例;受药品因素影响而出现差错14例,占28.00%,药品包装所致5例,药品名字相近3例,药品质量问题所致2例,因药品摆放缺乏规范所致4例。结论影响门诊药房中药调配差错原因复杂,应严格规范中药处方书写方法,提高调剂人员的专业水平,加强药材质量管理,以提高门诊药房中药调配质量。

Objective To analyze the causes of errors in dispensing Chinese medicines in outpatient pharmacies,and to explore preventive measures.Methods 50 cases of errors in the dispensing of traditional Chinese medicine prescriptions in the hospital from June 2016 to December 2018 were selected as the research objects.The dispensing situation of 50 cases was analyzed,and the reasons affecting the quality of traditional Chinese medicine dispensing were studied.Results Among the 50 cases of errors in traditional Chinese medicine dispensing selected by this research institute,20 cases were affected by prescription factors,accounting for 40.00%,10 cases were errors in prescription input,6 cases were errors in trial variance and 4 cases were neglected prescription footnotes;16 cases of errors were affected by dispensing factors,accounting for 32.00%,10 cases were errors in dosage adjustment and 4 cases were waste in the process of dispensing tablets,2 cases of drug omission;14 cases of errors caused by drug factors,accounting for 28.00%,5 cases of drug packaging,3 cases of similar drug names,2 cases of drug quality problems,the remaining 4 cases due to lack of standardization of drug placement.Conclusion The reasons for errors in dispensing Chinese medicines in outpatient pharmacies are complex.We should strictly standardize the writing methods of Chinese medicines prescriptions,improve the professional level of dispensing personnel,and strengthen the quality management of medicines in order to improve the dispensing quality of Chinese medicines in outpatient pharmacies.
