心血管疾病患者口服他汀药物依从性的相关因素分析

Analysis of related factors of oral statin compliance in patients with cardiovascular disease

目的分析心血管疾病患者口服他汀药物依从性的相关因素。方法采集口服他汀药物心血管疾病患者140例的相关资料,参考实施口服他汀药物依从性评判,比较依从性好及依从性差患者临床特征之间的差异。结果140例患者中口服依从性好90例(64.29%),依从性差50例(35.71%),依从性好组年龄≥70岁、未婚及丧配、居住地城镇、初中以下文化水平、认知不足、经济能力较差、服用药物种类≥3种、心理反应异常、缺少家庭支持的比例均低于依从性差组,差异均有统计学意义(P<0.05)。Logistic回归分析导致依从性差危险因素:对疾病及药物认知不足(OR=4.38,95%CI:1.09~17.28)、患者年龄≥70岁(OR=3.03,95%CI:1.14~18.05)、他汀药物不良反应(OR=3.94,95%CI:1.53~14.27)、经济收入低(OR=3.15,95%CI:1.32~15.38)。结论多种因素影响心血管疾病患者口服降压药的依从性,应针加强干预导致患者用药依从性差的主要原因,提高他汀药物口服依从性。

Objective To analyze the related factors affecting the compliance of patients with cardiovascular diseases to oral statins.Methods Data of 140 patients with cardiovascular diseases who took statins orally were collected.Implementation of oral statin compliance was evaluated.Compared the difference between the two groups of patients with good compliance and poor compliance.Results In 140 patients,90 patients(64.29%)had good oral compliance,and 50 patients(35.71%)had poor compliance.The proportion of age over 70 years old,unmarried,residence place town,cultural level below junior high school,understanding,economic capacity poor,drug types should be more than 3,psychological reaction abnormal,lack family support of good compliance group were lower than that of poor compliance group,the difference was statistically significant(P<0.05).Logistic regression analysis led to poor compliance risk factors:disease and inadequate drug awareness(OR=4.38,95%CI:1.09~17.28),the patient′s age over 70 years old(OR=3.03,95%CI:1.14~18.05),adverse drug reactions of statins(OR=3.94,95%CI:1.53~14.27),economic income was low(OR=3.15,95%CI:1.32~15.38).Conclusion Various factors can affect the compliance of patients with cardiovascular diseases with oral antihypertensive drugs.It is necessary to strengthen intervention to improve the compliance of patients with cardiovascular diseases with oral antihypertensive drugs.
