基于北京市2009-2016年卫生总费用的资源配置分析

Analysis on resource allocation of total health expenditure in Beijing from 2009 to 2016

基于2009年新医改以来北京市卫生总费用核算结果,用时间序列对比分析法,探讨北京市卫生资源配置在供给、需求以及供需总量等方面的成果与问题。结果显示,北京市2009-2016年卫生资源配置的主要成果包括:①卫生资源总量从2009年的936亿元增加到2016年的2709.61亿元(机构法);②卫生总费用占GDP的比重从2009年的5.67%上升至2016年的7.98%;③农村、城镇居民个人卫生支出占人均消费性支出比重从2009年的9.45%、6.35%下降到2016年的5.97%、4.22%;④社会卫生支出及占比从2009年的296.25亿元(42.96%)上升到2016年的1247.23亿元(60.87%);⑤2016年医院费用总额是2009年的3.05倍。存在的问题主要是:卫生资源总体配置不均衡、卫生资源需求结构的均衡性有待改善、卫生资源供给的均衡性有待优化。针对问题提出了相应建议,以期为制定相关政策提供参考。

Based on the results of total health expenditure accounting in Beijing since new health care reform in 2009,this paper discusses the achievements and problems of health resource allocation in Beijing in terms of supply,demand and total supply and demand with the method of time series comparative analysis.The results show that main achievements of health resource allocation in Beijing from 2009 to 2016 include as follow,1.Total health resources increased from 93.6 billion yuan in 2009 to 270.961 billion yuan in 2016(provider distribution method).2.The proportion of total health expenditure in GDP rose from 5.67% in 2009 to 7.98% in 2016.3.The proportion of rural and urban residents'personal health expenditure in per capita consumption expenditure decreased from 9.45%and 6.35% in 2009 to 5.97% and 4.22% in 2016.4.Social health expenditure and proportion increased from 29.625 billion yuan(42.96%)in 2009 to 124.723 billion yuan(60.87%)in 2016.5.The total cost of hospital in 2016 was 3.05 times of that in 2009.The main problems such as overall allocation of health resources is unbalanced,the balance of health resource demand structure needs to be improved,and the balance of health resource supply needs to be optimized.For these problems,the corresponding suggestions are put forward in order to provide reference for the formulation of relevant policies.
