第一掌骨桡背侧动脉皮支链带筋膜蒂岛状皮瓣修复术对拇指组织缺损修复的影响

目的分析第一掌骨桡背侧动脉皮支链带筋膜蒂岛状皮瓣修复拇指组织缺损对皮瓣的影响。方法将2017.01-2019.01期间我院收治的100例拇指组织缺损患者作为研究对象,随机分为对照组和观察组,各50例,观察组采用第一掌骨桡背侧动脉皮支链带筋膜蒂岛状皮瓣修复术进行治疗,对照组采用腹部皮瓣对拇指组织进行修复,对比两组DASH评分,手术指标。结果观察组DASH评分显著低于对照组,组间数据有统计学意义(P<0.05);观察组手术时间以及术中出血量均显著低于对照组,组间数据有统计学意义(P<0.05)。结论第一掌骨桡背侧动脉皮支链带筋膜蒂岛状皮瓣修复拇指组织缺损具有操作简单,易掌握等优势,且对于患者具有显著的疗效,属于修复拇指部分缺损的有效方式,使用价值高,值得推广。

Objective To study the effect of the first metacarpal and posterior branch of the dorsal metacarpal artery on the flap in the repair of the tissue defect of the thumb.Methods 100 cases of patients with thumb tissue defect and who treated in our hospital from January 2017 to January 2019 were selected as the research objects,and the patients were randomly divided into control group and observation group,with 50 cases in each group.The observation group treated with repair of the first dorsal radial metacarpal artery with fascial pedicle island flap,while the control group repair of bdominal flap to thumb.Then,the DASH score and operation indexes of two groups were compared.Results The DASH score in the observation group was significantly lower than that in the control group,and the data between groups were statistically significant(P<0.05);The operation time and bleeding volume in the observation group were significantly lower than those in the control group,and the data between the groups were statistically significant(P<0.05).Conclusion The repair of thumb tissue defect with the first dorsal radial metacarpal artery skin branch with fascial pedicle island flap which it has the advantages of simple operation and easy to master,and it is an effective way to repair the partial defect of thumb and has high value in use,thus it is worth promoting.
