血清CTRP-9、FKN水平与慢性阻塞性肺疾病患者急性加重的关系研究 被引量:14

摘要 目的探讨血清补体C1q/肿瘤坏死因子相关蛋白9(CTRP-9)、不规则趋化因子(FKN)水平与慢性阻塞性肺疾病患者急性加重的关系。方法选取2017年3月至2019年3月本院慢性阻塞性肺疾病患者122例,依据病情分为急性加重期组(n=50例)和稳定期组(n=72例),同期选取体检非慢性阻塞性肺疾病人员30例,所有人员均检测第1秒用力呼气容积占预计值百分比(FEV1%pred)及血清CTRP-9、FKN、白介素-1β(IL-1β)、肿瘤坏死因子α(TNF-α)水平。结果慢性阻塞性肺疾病患者FEV1%pred明显低于非慢性阻塞性肺疾病人员,慢性阻塞性肺疾病患者血清CTRP-9、FKN、IL-1β、TNF-α水平明显高于非慢性阻塞性肺疾病人员,差异有统计学意义(P<0.05);急性加重期组FEV1%pred明显低于稳定期组,急性加重期组血清CTRP-9、FKN、IL-1β、TNF-α水平明显高于稳定期组,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,慢性阻塞性肺疾病患者中,CTRP-9、FKN均与FEV1%pred呈负相关,但均与IL-1β、TNF-α呈正相关(P<0.05)。结论血清CTRP-9、FKN水平与慢性阻塞性肺疾病发生发展有关,其水平升高可能促进了患者肺部炎症反应损伤而导致急性加重的发生。 Objective To discuss the relationship between serum complement C1 q/tumor necrosis factor-related protein-9(CTRP-9),Fractalkine(FKN)levels and acute exacerbation in patients with chronic obstructive pulmonary disease.Methods 122 patients with chronic obstructive pulmonary disease in our hospital from March 2017 to March 2019 were selected,and they were divided into the acute exacerbation group(n=50 cases)and the stable period group(n=72 cases).30 non-chronic obstructive pulmonary disease patients were selected from physical examination during the same period as the control group.The percentage of forced expiratory volume in the first second(FEV1%pred)and serum CTRP-9,FKN,interleukin-1 beta(IL-beta),tumor necrosis factor alpha(TNF-alpha)were detected in all patients.Results The value of FEV1%pred was significantly lower,and the levels of serum CTRP-9,FKN,IL-beta and TNF-alpha were significantly higher in the stable group than in control group(P<0.05).The value of FEV1%pred was significantly lower in the acute group than in the stable period group,and the levels of serum CTRP-9,FKN,IL-beta and TNF-alpha in acute exacerbation group were significantly higher than those in the stable period group(P<0.05).The Pearson correlation analysis showed that,in chronic obstructive pulmonary disease patients,CTRP-9 and FKN were negatively correlated with FEV1%pred,but they were positively correlated with IL-beta and TNF-alpha(P<0.05).Conclusion Serum CTRP-9 and FKN levels are associated with the development of chronic obstructive pulmonary disease,and their elevation may promote the occurrence of acute exacerbation of pulmonary inflammation in patients with chronic obstructive pulmonary disease.
