应用血液净化联合前列地尔治疗难治性肾病综合征致急性肾衰竭的疗效观察

Clinical Observation of Hemopurification combined with Alprostadil in the treatment of acute renal failure caused by refractory nephrotic syndrome

目的:探讨应用血液净化联合前列地尔治疗难治性肾病综合征致急性肾衰竭的临床疗效。方法:选取2016年01月-2018年12月我院收治的难治性肾病综合征致急性肾衰竭患者60例作为研究对象,按随机数字表法分为观察组与对照组,其中对照组患者(n=29)给予血液净化治疗,观察组患者(n=31)在血液净化基础上联合前列地尔治疗(前列地尔20μg加入5%葡萄糖注射液中静滴,一天一次,持续治疗1周),比较两组患者治疗3 d、7 d时的肌酐、尿素、肌酐清除率、尿素氮、血液流变学、血钾及尿量。结果:治疗3 d、7 d时,观察组患者的肌酐、尿素、肌酐清除率、尿素氮、血液流变学、血钾及尿量等指标改善均明显优于对照组患者(P<0.05)。结论:对难治性肾病综合征致急性肾衰竭患者应用血液净化联合前列地尔治疗可以有效提高治疗效果,且安全性较高,利于患者的预后。

Objective:To investigate the clinical effects of blood purification combined with Alprostadil in the treatment of acute renal failure caused by refractory nephrotic syndrome.Methods:60 patients with acute renal failure caused by refractory nephrotic syndrome admitted to our hospital from January 2016 to December 2018 were enrolled.According to the random number table method,31 cases were assigned into the observation group and 29 cases into the control group.The observation group was given hemopurification combined with Alprostadil,while the control group was given hemopurification.The Cr,UR,CCR,BUN,hemorheology,potassium,and urine output at day 3 and day 7 in the treatment were measured and compared.Results:During treatment at 3d and 7d,the improvement of Cr,UR,CCR,BUN,hemorheology,potassium,and urine volume in the control group was significantly worse than those in the observation group(P<0.05).Conclusion:Blood purification combined with Alprostadil can effectively improve the therapeutic effects on refractory nephrotic syndrome patients with acute renal failure.
