组织激肽释放酶8与原发性高血压严重程度的相关性 被引量:3

摘要 目的探讨组织激肽释放酶(KLK)8水平与原发性高血压严重程度的相关性。方法选取原发性高血压患者324例(原发性高血压组)、体检健康者100名(正常对照组)。检测所有对象血清KLK8水平,同时测量心室厚度相关指标[室间隔厚度(LVST)、左心室后壁厚度(LVPWT)、左心室舒张末内径(LVDD)]。按血压升高的程度将原发性高血压患者分为高血压1级(98例)、2级(126例)和3级(100例),记录所有患者的心血管危险因素。结果高血压2级组和3级组血清KLK8水平明显高于正常对照组(P<0.05),而高血压1级组与正常对照组之间差异无统计学意义(P>0.05)。血清KLK8水平随血压分级的增高而升高(P<0.001)。血清KLK8水平与LVST、LVPWT和LVDD呈正相关(r值分别为0.627、0.586、0.543,P<0.001)。伴左心室肥厚的高血压2级和3级患者血清KLK8水平明显高于同级不伴左心室肥厚的患者(P<0.05)。同级高血压患者血清KLK8水平随心血管危险分层的增加而升高(P<0.05)。结论KLK8或可作为评估原发性高血压患者心血管风险和心室肥厚的潜在指标。 Objective To study the relationship between kallikrein-related peptidase(KLK)8 concentration and the severity of primary hypertension.Methods A total of 324 primary hypertension patients(primary hypertension group)and 100 healthy subjects(healthy control group)were enrolled.Serum KLK8 levels and relevant indexes of ventricular hypertrophy[left ventricular septal thickness(LVST),left ventricular posterior wall thickness(LVPWT)and left ventricular end diastolic diameter(LVDD)]were determined.The primary hypertension group was classified into 3 subgroups by blood pressure levels,grade 1(98 cases),grade 2(126 cases)and grade 3(100 cases)groups.Cardiovascular risk factors were recorded.Results The serum KLK8 concentrations in grade 2 and grade 3 groups were higher than that in healthy control group(P<0.05),but there was no statistical significance between grade 1 and healthy control groups(P>0.05).With the progress of the diseases from grade 1 to grade 3,the serum KLK8 concentrations increased correspondingly(P<0.001).Serum KLK8 concentration was positively correlated to LVST,LVPWT and LVDD(r=0.627,0.586 and 0.543,respectively,P<0.001).Serum KLK8 concentrations in grade 2 and grade 3 groups with left ventricular hypertrophy were higher than those without left ventricular hypertrophy(P<0.05).With the progress of cardiovascular risk stratification from low to very high,the serum KLK8 concentrations increased correspondingly(P<0.05).Conclusions KLK8 may be a potential biomarker for assessing cardiovascular risk and ventricular hypertrophy in primary hypertension patients.
