超薄不备牙全瓷贴面修复前牙及前磨牙的存留率及其影响因素分析

Retention Rate of Anterior and Premolar Teeth Repaired with Ultra-Thin Porcelain Veneers and Its Influencing Factors

目的观察超薄不备牙全瓷贴面修复前牙及前磨牙的留存率,同时探讨其影响因素。方法回顾性分析2015-2016年在广州市东升医院口腔科接受超薄不备牙全瓷贴面修复前牙及前磨牙80例(共450颗牙)患者的临床资料,于治疗后第6、12、18、24、30个月随访,观察患者前牙和前磨牙修复的留存率,同时采用Logistic回归分析对修复效果的影响因素进行分析。结果 80例患者治疗后完成随访78例(共440颗牙),治疗后第6、12、18、24、30个月患者前牙和前磨牙修复留存率分别为100.0%、97.7%、95.5%、93.2%、90.9%,整体修复效果较好。Logistic回归分析显示,使用不当、不良习惯、前伸颌接触或正中颌接触、重度氟斑牙或重度四环素牙或重着色牙是影响修复效果的主要危险因素(OR=4.872、5.416、2.710、3.019,P=0.032、0.030、0.022、0.005)。结论超薄不备牙全瓷贴面修复前牙及前磨牙的长期留存率较高,效果良好,而可能导致修复失败的主要影响因素是使用不当、不良习惯等,需要给予高度重视。

Objective To investigate the retention rate of anterior and premolar teeth repaired with ultra-thin porcelain veneers and its influencing factors.Methods Clinical data of 80 patients(450 teeth) who received anterior and premolar tooth repair in the Department of Stomatology of Guangzhou Dongsheng Hospital from January 2015 to December 2016 were analyzed retrospectively.The retention rate of anterior and premolar teeth was observed after 6,12,18,24 and 30 months of follow-up.The factors influencing restoration were analyzed using logistic regression analysis.Results A total of 78 patients(440 teeth) completed the follow-up.The retention rates of anterior and premolar teeth were 100.0%,97.7%,95.5%,93.2% and 90.9% at 6,12,18,24 and 30 months after treatment,respectively.Logistic regression analysis showed that the main risk factors affecting repair efficacy were improper use,bad habits,anterior or median jaw contact,and severe dental fluorosis,tetracycline pigmentation or re-stained teeth(OR=4.872,5.416,2.710 and 3.019,respectively;P=0.032,0.030,0.022 and 0.005,respectively.).Conclusion Ultra-thin porcelain veneer repair can achieve a high retention rate of anterior and premolar teeth and a good repair efficacy.However,improper use,bad habits and other factors may result in the failure of repair,which needs more attention.
