摘要
目的研究妊娠早期甲状腺功能减退对子代发育的影响,为临床诊疗提供参考依据。方法收集143例早孕建卡孕妇作为研究对象,按照孕妇的甲功情况及治疗意愿将其分为三组,其中正常孕妇组42例,甲状腺功能减退异常治疗组60例,甲状腺功能减退异常未治疗组41例。对三组婴儿进行随访,比较三组婴儿分娩时情况及10月龄CDCC婴幼儿发育量表评分。结果①在出生体重和身长方面,三组胎儿差异均无统计学意义(P>0.05)。②在出生胎龄方面,出生胎龄小于37周的胎儿数正常组及异常治疗组差异无统计学意义(P>0.05),但均少于异常未治疗组,差异有统计学意义(P<0.05)。③在智能发育指数方面,甲功异常治疗组和未治疗组均明显低于甲功正常组,差异有统计学意义(P<0.05);甲功异常治疗组和未治疗组之间差异无统计学意义。④在运动发育指数方面,甲功异常治疗组与正常组相比差异无统计学意义(P>0.05),但均高于甲功异常未治疗组,差异有统计学意义(P<0.05)。结论妊娠早期甲状腺功能减退会对子代的发育产生不良影响,应于孕前或孕早期及时筛查和干预。
Objective To study the effect of hypothyroidism in early pregnancy on offspring’s behavior development,in order to provide reference for clinical diagnosis and treatment.Methods 143 cases of pregnant women with early pregnancy were enrolled into the study.Their cards were collected and divided into three groups according to the condition of thyroid function and treatment intention,including 42 cases of normal pregnant women,60 cases in the hypothyroidism abnormal treatment group and 41 cases in the hypothyroidism abnormal non-treatment group.Infants from these three groups were followed up to compare the situation at delivery and the score of CDCC infant development scale at 10 months old.Results ①There was no significant difference in birth weight and birth length among the three groups(P>0.05).②In terms of gestational age,there was no significant difference between the normal group and the abnormal treatment group(P>0.05),but there was significant difference between the normal group and the abnormal treatment group(P<0.05).③In terms of intelligence development index,the treatment group and the non-treatment group were significantly lower than the normal group(P<0.05),while there was no significant difference between the treatment group and the non-treatment group.④ In the aspect of sports development index,there was no significant difference between the treatment group and the normal group(P>0.05),but both groups were higher than that of the untreated group(P<0.05).Conclusion Hypothyroidism in early pregnancy might have adverse effects on the development of offspring,so timely screening and intervention should be carried out before or during early pregnancy.
