摘要
目的探讨实时荧光PCR法在手足口病病毒检测中应用效果。方法选取2017年3月至2018年12月在我院接受治疗的疑似手足口病患儿93例,均实施荧光PCR法及病毒分离培养法检测肠道病毒,以临床表现、实验室检查、血清及病原学综合诊断结果为金标准,对比两种不同检测方法诊断结果、诊断价值,并分析实施荧光PCR检查肠道病毒71型(human enterovirus 71,EV71)、A组柯萨奇病毒(coxsachie virus A16,CoxA16)、其他肠道病毒(EV)检出情况。结果经临床表现、实验室检查、血清及病原学综合诊断,93例疑似病例中手足口病患儿53例。实时荧光PCR法检测手足口病的准确性、灵敏度、特异性、阳性预测值及阴性预测值均高于病毒分离培养法,差异有统计学意义(P<0.05);实时荧光PCR法检查EV71阳性检出率48.84%,Cox A16阳性检出率9.30%,其他EV阳性检出率41.86%。结论在手足口病病毒检测中实时荧光PCR法具有较高应用价值,可有效提高临床诊断的准确性、灵敏度及特异性。
Objective To investigate the applicable effect of real-time fluorometric PCR in detection of hand-foot-mouth disease(HFMD)virus.Methods 93 children with suspected HFMD treated in our hospital from March,2017 to December,2018 were selected for the study,and they were detected for enterovirus by real-time fluorometric PCR,as well as virus isolation and culture.The clinical manifestation,laboratory examination,serum and etiological diagnosis results were combined as the gold standard.The diagnostic results and value of two detection methods were compared.The detection results of human enterovirus 71(EV71),Coxsachie virus A16(CoxA16),and other enterovirus(EV)by fluorometric PCR were analyzed.Results According to clinical manifestation,laboratory examination,serum and etiological diagnosis,there were 53 cases of HFMD in 93 suspected cases.The accuracy,sensitivity,specificity,positive predictive value,and negative predictive value of real-time fluorometric PCR in the diagnosis of HFMD were higher than those of virus isolation and culture(P<0.05).The positive detection rate of EV71,Cox A16 and other EV by real-time fluorometric PCR were 48.84%,9.30% and 41.86%,respectively.Conclusion Real-time fluorometric PCR in the detection of HFMD has a relatively higher application value,and it can effectively increase the diagnostic accuracy,sensitivity and specificity.
