Management effect of family doctors’ contracted service team on the low-income population with hypertension and type 2 diabetes

目的:研究家庭医生签约服务团队对高血压、2型糖尿病低收入人群的管理效果.方法:家庭医生签约服务团队成员对入选高血压、2型糖尿病低收入人群进行强化健康管理;以同期金海湖镇社区卫生服务中心高血压、2型糖尿病健康管理血压、血糖达标率为对比.结果:1年强化健康管理后,低收入人群与同期金海湖镇社区卫生服务中心高血压、糖尿病患者血压、血糖达标率比较有明显提高;且1年强化管理后低收入人群血压、血糖达标率明显高于同期金海湖镇社区卫生服务中心.结论:家庭医生签约服务团队对高血压、2型糖尿病低收入人群的管理效果显著.

Objective:To explore the management effect of family doctors'contracted service team on the low-income population with hypertension and type 2 diabetes.Methods:Members of the family doctors'contracted service team strengthen the health management of the selected low-income patients with hypertension and type 2 diabetes.The compliance rate of blood pressure and blood glucose of hypertension and type 2 diabetes after health management in Community Health Service Center of Jinhaihu Town in the same period was compared.Results:One year after strengthening health management,compared with the compliance rate of blood pressure and blood glucose of hypertension and type 2 diabetes after health management in Community Health Service Center of Jinhaihu Town,the standard rate of low-income people had been significantly improved.After one year of strengthening management,the compliance rate of blood pressure and blood glucose in low-income population was significantly higher than that in Community Health Service Center of Jinhaihu Town in the same period.Conclusion:The management effect of family doctors'contracted service team on the low-income population with hypertension and type 2 diabetes was significant.
