跟骨骨折患者行跗骨窦小切口空心钉内固定治疗的临床效果

Clinical effect of cannulated screw internal fixation through tarsal sinus small incision on patients with calcaneal fracture

目的探析跗骨窦小切口空心钉内固定术治疗跟骨骨折的有效性和安全性。方法选取2015年7月至2018年6月我院收治的跟骨骨折患者120例,依据随机数字表将其分为对照组和研究组,每组60例。对照组行微型锁定内固定术治疗,研究组行跗骨窦小切口空心钉内固定术治疗,并进行3个月随访。观察两组患者术后并发症发生情况,治疗前和随访结束后的Bohler角和Gissane角,随访结束后的骨折愈合情况及后足功能。结果研究组术后并发症总发生率显著低于对照组(P<0.05)。随访结束后,两组Bohler角明显增大,Gissane角明显减少,且研究组优于对照组(P<0.05)。随访结束后,研究组骨折愈合评分和后足功能优良率显著高于对照组(P<0.05)。结论跗骨窦小切口空心钉内固定术治疗跟骨骨折,不仅可减少术后并发症,促进患者解剖结构复位和骨折愈合,还可改善患者的后足功能,有效性和安全性均较高,值得在临床上推广应用。

Objective To explore the effectiveness and safety of cannulated screw internal fixation through tarsal sinus small incision in the treatment of calcaneal fracture.Methods A total of 120 cases of calcaneal fracture patients admitted in our hospital from July 2015 to June 2018 were selected.According to the random number table,the patients were divided into control group and study group,with 60 cases in each group.The control group was treated with micro-locking internal fixation,while the study group was treated with cannulated screw internal fixation through tarsal sinus small incision,and all were followed up for 3 months.Postoperative complications,Bohler angle and Gissane angle before treatment and after follow-up,fracture healing and hind foot function after follow-up of the two groups were observed.Results The total incidence of postoperative complications in the study group was significantly lower than that in the control group(P<0.05).At the end of follow-up,Bohler angle of the two groups significantly increased,Gissane angle significantly reduced,and those of the study group were better than the control group(P<0.05).At the end of follow-up,the fracture healing score and the excellent and good rate of hind foot function in the study group were significantly higher than those in the control group(P<0.05).Conclusion Cannulated screw internal fixation through tarsal sinus small incision for treating calcaneal fractures can not only reduce postoperative complications,promote the reduction of the patients’anatomical structure and fracture healing,but also improve the patients’hind foot function,with high effectiveness and safety,which is worthy of clinical application and promotion.
