摘要
目的探讨高频超声诊断分析早期乳腺癌及腋窝转移性淋巴结的效果。方法选取2017年5月—2018年11月福建医科大学附属泉州第一医院收治的乳腺癌患者198例,全部患者均接受乳腺与腋窝淋巴结高频超声检查,以病理检查作为诊断“金标准”,根据是否发生腋窝淋巴结转移分为转移组(n=105)与未转移组(n=93)。评估高频超声诊断分析早期乳腺癌及腋窝转移性淋巴结的准确性。结果相较于未转移组,转移组皮质向心性厚度增加、淋巴结内钙化灶数量、淋巴结长短径比值>2、淋巴结边界模糊率均更高,差异有统计学意义(P<0.05);转移组血流分布主要为周边型,分级为3级,未转移组血流分布主要为门型,分级为2级;乳腺癌及腋窝转移性淋巴结诊断中高频超声的灵敏度为90.48%,特异度为81.72%,准确率为86.36%。结论高频超声可有效诊断分析早期乳腺癌及腋窝转移性淋巴结情况,为疾病治疗提供参考。
Objective To investigate the effect of high frequency ultrasound in the diagnosis and analysis of early breast cancer and axillary metastatic lymph nodes.Methods 198 patients with breast cancer admitted to Quanzhou First Hospital Affiliated to Fujian Medical University from May 2017 to November 2018 were selected.All patients underwent high frequency ultrasound examination of breast and axillary lymph nodes.Pathological examination was used as the"golden standard"for diagnosis.They were divided into metastatic group(n=105)and non-metastatic group according to whether axillary lymph node metastasis occurred.(n=93).To evaluate the accuracy of high frequency ultrasound in diagnosing early breast cancer and axillary metastatic lymph nodes.Results Compared with the non-metastasis group,the cortical centripetal thickness increased,the number of calcified lesions in lymph nodes,the ratio of length to diameter of lymph nodes>2,and the blurring rate of lymph node boundary were higher in the metastasis group,the difference is statistically significant.(P<0.05);the blood flow distribution in the metastasis group was mainly peripheral type,graded as grade 3,and in the non-metastasis group was mainly portal type,graded as grade 2 breast;The sensitivity,specificity and accuracy of high frequency ultrasound in the diagnosis of adenocarcinoma and axillary metastatic lymph nodes were 90.48%,81.72% and 86.36%,respectively.Conclusion High-frequency ultrasound can effectively diagnose and analyze early breast cancer and axillary metastatic lymph nodes,and provide reference for disease treatment.
