摘要
目的:探讨超声造影在诊断周围型肺癌中的价值。方法:对44例周围型肺癌患者进行超声造影检查,按照病理类型分为腺癌、鳞癌和转移癌3组,统计并比较三组患者的造影剂增强程度、分布模式、开始增强时间和到达峰值的时间。结果:腺癌患者中,高增强13例,低增强3例,开始增强时间为(9.3±1.8)s,到达峰值时间为(18.7±3.2)s;鳞癌患者中,高增强4例,低增强10例,开始增强时间为(9.9±2.7)s,到达峰值时间为(20.1±3.6)s;在转移癌中,高增强2例,低增强11例,无增强1例,开始增强时间为(23.2±4.1)s,到达峰值时间为(35.8±4.3)s。腺癌检查结果为高增强和均匀增强的例数显著多于鳞癌和转移癌(P<0.05),转移癌的开始增强时间和到达峰值时间显著晚于腺癌和鳞癌(P<0.05)。结论:超声造影用于诊断周围型肺癌具有安全、快速、操作简便的特点,是一种值得推广的影像学检查方法。
Objective: To explore the value of contrast enhanced ultrasound in the diagnosis of peripheral lung cancer.Methods: Forty-four patients with peripheral lung cancer were examined by ceus and divided into three groups according to their pathological types: adenocarcinoma, squamous cell carcinoma and metastatic carcinoma.Results: Among the patients with adenocarcinoma, there were 13 patients with high enhancement, 3 patients with low enhancement, the initial enhancement time was(9.3±1.8) s, and the peak time was(18.7±3.2) s.Among the patients with squamous cell carcinoma, there were 4 cases with high enhancement, 10 cases with low enhancement, and the initial enhancement time was(9.9±2.7) s, and the peak time was(20.1±3.6) s.In metastatic cancer, there were 2 patients with high enhancement, 11 patients with low enhancement, and 1 patient with no enhancement. The initial enhancement time was(23.2±4.1) s, and the peak time was(35.8±4.3) s.The number of cases of adenocarcinoma with high enhancement and uniform enhancement was significantly higher than that of squamous cell carcinoma and metastatic cell carcinoma(P<0.05), and the initial enhancement time and peak time of metastatic cell carcinoma were significantly later than those of adenocarcinoma and squamous cell carcinoma(P<0.05).Conclusion: Contrast-enhanced ultrasound(ceus) is a safe, rapid and simple method for the diagnosis of peripheral lung cancer.
