EpCAM在前列腺癌中的表达及其相关临床意义

摘要 目的:研究上皮特异性黏附分子(EpCAM)在正常前列腺组织、前列腺癌组织和骨转移性前列腺癌组织中的表达情况,并探讨其表达水平与临床病理特征及预后的关系。方法:结合公共数据库中的前列腺癌资料,分析EpCAM的mRNA和蛋白质表达水平。收集2014-2018年深圳市南山区人民医院及北京大学深圳医院正常前列腺标本18例、原发性前列腺癌71例、骨转移性前列腺癌标本12例。采用免疫组化EnVision法检测EpCAM蛋白在不同组织内的表达,并分析EpCAM的表达水平与前列腺腺癌临床病理特征的关系。结果:TCGA数据库、Human Protein Atlas数据库的分析结果显示,正常前列腺组织中EpCAM的mRNA和蛋白质水平明显低于前列腺癌组织(P<0.05);EpCAM在不同种族、年龄及Gleason评分组之间前列腺癌组织中表达比较,差异均无统计学意义(P>0.05);与正常前列腺组织相比,前列腺癌FLI1+亚型和IDH1突变亚型的EpCAM表达未明显增加(P>0.05);前列腺癌组织内EpCAM的DNA启动子序列甲基化水平低于正常前列腺组织(P<0.05),但不同年龄组前列腺癌之间的甲基化水平比较,差异均无统计学意义(P>0.05)。免疫组化检测结果显示,前列腺癌组织中EpCAM蛋白表达水平高于正常前列腺组织(P<0.05),尤其在前列腺癌骨转移组织表达更高(P<0.05);在不同pT分期及Gleason评分的前列腺癌中EpCAM的蛋白水平比较差异均有统计学意义(P<0.05),而脉管及神经束侵犯组与其未侵犯组EpCAM蛋白表达水平比较,差异均无统计学意义(P>0.05)。结论:前列腺癌中EpCAM的mRNA及蛋白水平均高于正常前列腺组织,EpCAM的表达水平与前列腺癌分子分型、临床T分期、Gleason评分和及骨转移均密切相关,EpCAM的表达上调可能参与了前列腺癌的发生、进展和骨转移等过程。 Objective:To investigate the expression of epithelial-specific adhesion molecule(EpCAM)in normal prostate tissue,prostatic carcimoma tissue and bone metastatic prostatic carcinoma tissue,and the correlations between the expression of EpCAM and its associated clinical data.Method:Combined with prostatic carcinoma data from public databases,mRNA and protein expression levels of EpCAM were analyzed.EnVision method was used to detected the expression of EpCAM in comprehensive tissue studies including 18 normal prostate specimens,71 primary prostatic carcimoma tissues and 12 metastatic lesions between 2014 and 2018 at the Department of Pathology of Shenzhen Nanshan People’s Hospital and Peking University Shenzhen Hospital.Statistically analyze the correlation between EpCAM protein and the clinicopathological parameters.Result:The data from TCGA and Human Protein Atlas database showed that the mRNA and protein levels of EpCAM in normal prostate tissues were significantly lower than that in prostatic carcinoma tissues(P<0.05).There was no significant difference in the expression of EpCAM in prostatic carcinoma among different races,ages and Gleason groups(P>0.05).Compared with normal prostate tissue,EpCAM expression in FLI1+and IDH1 mutant subtypes of prostatic carcinoma were not significantly increased(P>0.05).EpCAM DNA promoter sequence methylation level in prostatic carcinoma tissue was lower than that in normal prostate tissue(P<0.05),without significant difference between different age groups(P>0.05).Immunohistochemistry showed that the expression level of EpCAM protein in prostatic arcinoma tissue was higher than that in normal prostate tissue(P<0.05),especially higher in prostatic carcinoma bone metastatic tissue(P<0.05).EpCAM protein expression had significantly difference in different pT stages and Gleason scores of prostatic carcinoma(P<0.05),with no significant correlation with vascular invasion and nerve invasion(P>0.05).Conclusion:The mRNA and protein levels of EpCAM in prostatic carcinoma were higher than those in normal prostate tissue.The expression of EpCAM is closely related to the molecular typing,clinical T stage,Gleason score and bone metastasis of prostatic carcinoma.The up-regulated expression of EpCAM may be involved in the occurrence,progression and bone metastasis of prostatic carcinoma.
