良恶性胸腔积液患者VEGF、EGFR和MMP-9水平及相关因素分析

摘要 目的:探究良恶性胸腔积液患者血管内皮生长因子(VEGF)、表皮生长因子受体(EGFR)及基质金属蛋白酶-9(MMP-9)水平及相关因素分析。方法:选取2017年9月-2018年12月在本院接受治疗的恶性胸腔积液患者68例作为恶性胸腔积液组,同期在本院进行治疗的良性胸腔积液患者57例作为良性胸腔积液组。获取两组患者的胸腔积液标本,采用酶联免疫吸附法测定其中VEGF、EGFR、MMP-9及肿瘤标志物的含量,采用荧光定量PCR法测定MDIG、WWOX、p53基因的表达量。进一步采用Pearson检验评估胸腔积液VEGF、EGFR、MMP-9含量对良恶性胸腔积液的鉴别价值。结果:恶性胸腔积液组患者胸腔积液中VEGF、EGFR及MMP-9的含量高于良性胸腔积液组患者;肿瘤标志物癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原153(CA153)、糖类抗原199(CA199)的含量高于良性胸腔积液组患者;胸腔积液中MDIG mRNA的表达量高于良性胸腔积液组患者,WWOX、p53 mRNA的表达量显著低于良性胸腔积液组患者,差异均有统计学意义(P<0.05)。经Pearson检验发现,胸腔积液中VEGF、EGFR、MMP-9的含量与其恶性程度直接相关。结论:恶性胸腔积液中VEGF、EGFR、MMP-9的含量上升,且其具体含量与胸腔积液恶性程度直接相关,可作为鉴别良恶性胸腔积液的可靠指标。 Objective:To investigate the levels of vascular endothelial growth factor(VEGF),epidermal growth factor receptor(EGFR)and matrix metalloproteinase-9(MMP-9)in patients with benign and malignant pleural effusion and analyze the related factors.Method:A total of 68 cases of malignant pleural effusion patients in our hospital during September 2017 to December 2018 were chosen as malignant pleural effusion group,57 cases of benign pleural effusion patients were chosen as benign pleural effusion group.The pleural fluid samples in two groups were obtained,serum contents of VEGF,EGFR,MMP-9 and tumor markers were determined by enzyme linked immunosorbent assay.The expressions of MDIG,WWOX and p53 genes were measured by fluorescent quantitative PCR.Pearson assay was used to evaluate the differential value of vascular endothelial growth factor(VEGF),epidermal growth factor receptor(EGFR)and matrix metalloproteinase-9(MMP-9)in benign and malignant pleural effusion.Result:Serum contents of VEGF,EGFR and MMP-9 in malignant pleural effusion group were higher than those in benign pleural effusion group;tumor markers such as CEA,CA125,CA153,CA199 were higher than those in benign pleural effusion group;the expression of MDIG mRNA in malignant pleural effusion was higher than that in benign pleural effusion group,expression of WWOX and p53 mRNA were significantly lower than those in benign pleural effusion group,the differences were statistically significant(P<0.05).The Pearson test showed that the contents of VEGF,EGFR and MMP-9 in pleural fluid were directly related to the degree of malignancy.Conclusion:Serum contents of VEGF,EGFR and MMP-9 in malignant pleural effusion increase,and their specific contents are directly related to the malignant degree of pleural effusion.It can be used as a reliable index to distinguish benign and malignant pleural effusion.
