传统切开和小切口内固定治疗交通意外致胫骨平台骨折临床疗效对比

Comparison of Efficacy and Safety of Traditional Incision Internal Fixation and Mini-incision Internal Fixation in the Treatment of Tibial Plateau Fracture Caused by Traffic Accident

目的:分析传统切开内固定术和小切口固定术治疗交通意外致胫骨平台骨折疗效及安全性。方法:选取本院2016年8月-2018年8月收治的90例交通意外导致的胫骨平台骨折患者的临床资料进行回顾性分析,根据资料中患者采用的手术方法将其分为观察组和对照组,每组各45例。观察组采用小切口内固定术治疗,对照组采用传统切开内固定术治疗。比较两组患者治疗效果、手术指标、并发症及生活质量评分。结果:观察组总有效率88.89%明显高于对照组66.67%,两组比较差异有统计学意义(P<0.05)。观察组术中出血量、术后引流量与对照组相比均明显减少(P<0.05),且下床活动时间也明显缩短(P<0.05),但观察组手术时间与对照组比较明显延长(P<0.05)。观察组患者并发症发生率8.89%明显低于对照组26.67%,两组比较差异有统计学意义(P<0.05)。治疗前,两组患者生活质量评分比较,差异均无统计学意义(P>0.05);治疗后,两组患者的躯体功能、机体疼痛、活力、情绪角色、社会功能以及心理健康评分均明显高于治疗前(P<0.05),且观察组治疗后的躯体功能、机体疼痛、活力评分明显高于对照组(P<0.05)。结论:采用小切口内固定术治疗交通意外致胫骨平台骨折其治疗效果较好,能明显降低并发生情况,提高患者生活质量,安全可靠。

Objective:To investigate efficacy and safety of traditional incision internal fixation and miniincision internal fixation in the treatment of tibial plateau fracture caused by traffic accident.Method:The clinical data of 90 patients with tibial plateau fractures caused by traffic accidents admitted to our hospital from August 2016 to August 2018 were retrospectively analyzed.According to the operative methods adopted by the patients,they were divided into observation group and control group,45 cases in each group.The observation group received the miniincision surgery,while the control group received the traditional incision internal fixation.Then the clinical efficacy,surgical status,complications,and quality of life were compared between the two groups.Result:The total effective rate of the treatment group was significantly higher than that of the control group(88.89%vs 66.67%,P<0.05).The amount of intraoperative blood loss and postoperative drainage in the observation group were significantly lower than those of the control group(P<0.05).The time of getting out of bed was also significantly shortened in the observation group(P<0.05),while the operation time of the observation group was significantly longer than that of the control group(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group(8.89%vs 26.67%,P<0.05).The quality of life had no difference before treatment between groups(P>0.05).After treatment,the score of physical function,body pain,vitality,emotional role,social function and mental health in two groups were all significantly increased(P<0.05),and they were higher in the observation group than those in the control group(P<0.05).Conclusion:The application of mini-incision internal fixation is a safe and effective way to reduce the incidence rate of complications and improve the quality of life in the treatment of tibial plateau fracture caused by traffic accident.
