摘要
目的探究不同血液净化方式对尿毒症患者疗效及生活质量的影响。方法选取本院2017年1月至2018年1月收治的尿毒症患者82例为研究对象,采用随机数字表法分对照组和观察组,每组41例。所有患者均采用相同透析机行血液净化,对照组实施普通透析[膜面积1.3 m^2,超滤系数13 ml/(h·mmHg)],观察组采用高通透析[膜面积1.8 m^2,超滤系数59 ml/(h·mmHg)],对比两组疗效、症状改善及生活质量评分差异。结果治疗前两组患者各项数据差异均无统计学意义,但观察组治疗后1个月、3个月甲状旁腺激素(PTH)、血钙、血磷、皮肤瘙痒评分、肌肉酸痛评分、关节疼痛评分低于对照组,SF-36评分高于对照组,差异均有统计学意义(P<0.05)。结论高通量血液透析治疗尿毒症效果显著优于普通血液透析,治疗3个月后患者血钙、血磷、PTH水平明显降低。
Objective To explore the effect of different blood purification methods on the curative effect and quality of life of uremic patients.Methods 82 uremic patients admitted to our hospital from January 2017 to January 2018 were selected as research objects, and were randomly divided into control group and observation group, with 41 cases in each group. All patients were treated with the same dialysis machine for blood purification. The control group was treated with ordinary dialysis [membrane area 1.3 m^2, ultrafiltration coefficient 13 ml/(h ·mmHg)], while the observation group was treated with high-flux dialysis [membrane area 1.8 m^2, ultrafiltration coefficient 59 ml/(h · mmHg)]. The curative effect, symptom improvement and quality of life scores of the two groups were compared. Results There was no significant difference between the two groups before treatment. However, the parathyroid hormone(PTH), blood calcium, blood phosphorus, skin pruritus score, muscle soreness score and joint pain score in the observation group were lower than those in the control group after one month and three months treatment, and the SF-36 score was higher than that in the control group, the differences were statistically significant(P<0.05). Conclusion The effect of high-flux hemodialysis on uremia is significantly better than that of ordinary hemodialysis. After 3 months of treatment, the levels of blood calcium, blood phosphorus and PTH in patients are significantly reduced.
