耳内镜下鼓膜置管联合腺样体切除术治疗儿童分泌性中耳炎临床疗效分析

Clinical analysis of endoscopic tympanic membrane combined with adenoidectomy for children with secretory otitis media

目的探讨耳内镜下鼓膜置管联合腺样体切除术治疗儿童分泌性中耳炎临床疗效。方法选取2016年12月至2017年12月本院诊治的分泌性中耳炎患儿106例为研究对象,随机分为对照组和研究组,每组53例。对照组只采取耳内镜下鼓膜置管术治疗,研究组采取耳内镜下鼓膜置管联合腺样体切除术治疗者。对两组患儿的中耳炎整体治疗效果、一年后复发率以及治疗前后语频区气导平均听阈变化进行分析。结果研究组患儿中耳炎整体治疗有效率(96.23%)高于对照组(84.91%);治疗后,两组患儿的语频区气导平均听阈水平均下降,差异均具有统计学意义(P<0.05),其中研究组语频区气导平均听阈低于对照组,差异均具有统计学意义(P<0.05);研究组的分泌性中耳炎复发率(1.89%)低于对照组(15.09%),差异均具有统计学意义(P<0.05)。结论耳内镜下鼓膜置管联合腺样体切除术治疗儿童分泌性中耳炎的整体疗效确切,可有效纠正患儿语频区气导平均听阈指标,且有根治性优势,值得推广。

Objective To investigate the clinical efficacy of endoscopic tympanic membrane combined with adenoidectomy in the treatment of children with secretory otitis media. Methods A total of 106 children with secretory otitis media were enrolled in the hospital from December 2016 to December 2017. All patients underwent surgery. The patients who underwent endoscopic tympanic membrane catheterization were used as controls. Who underwent endoscopic tympanic membrane combined with adenoidectomy as the study group(n=53), the overall treatment effect of otitis media in two groups, and the recurrence rate after one year statistical analysis was performed on the changes in the mean hearing threshold of the air conduction frequency before and after treatment. Results The overall treatment efficiency of otitis media in the study group was 96.23% higher than that in the control group which was 84.91%, the differences were statistically significant(P<0.05). After treatment, the average hearing threshold of the frequency band in the two groups decreased, in which the mean hearing index of the air frequency in the study group was lower than that in the control group, the differences were statistically significant(P<0.05). The recurrence rate of secretory otitis media in the study group was 1.89%lower than the control group which was 15.09%, the differences were statistically significant(P<0.05). Conclusion The overall efficacy of endoscopic tympanic membrane combined with adenoidectomy for children with secretory otitis media is accurate. It can effectively correct the average hearing threshold of speech frequency in children, and has a curative advantage. It is worth learning.
