延续性护理对支气管哮喘-慢性阻塞性肺疾病重叠综合征患者吸入治疗和家庭氧疗依从性的影响

Effects of continuous nursing care on inhalation therapy and domiciliary oxygen therapy compliance in patients with bronchial asthma-chronic obstructive pulmonary disease overlap syndrome

目的探讨延续性护理对支气管哮喘-慢性阻塞性肺疾病重叠综合征(ACOS)患者吸入治疗和家庭氧疗依从性的影响。方法选取2016年8月~2018年1月稳定期的ACOS患者71例,随机分为对照组与实验组,对照组36例,实验组35例。其中,对照组患者接受常规护理,实验组患者在对照组基础上接受3个月延续性护理,包括多次电话随访指导患者吸入治疗和家庭氧疗,同时使用微信平台发送教育视频,答疑解惑等,同时做好饮食、运动及病情监测及心理护理等,对比3个月后两组患者吸入治疗依从性、家庭氧疗依从性、干预前后生活质量评分。结果两组患者干预后,实验组吸入治疗依从性及家庭氧疗依从性高于对照组(P<0.05)。两组患者干预后圣乔治呼吸问卷(SGRQ)评分,实验组低于对照组(P<0.05)。结论采用延续性护理能有效提高支气管哮喘-慢性阻塞性肺病重叠综合征患者吸入治疗和家庭氧疗依从性,改善患者生活质量。

Objective To investigate the effects of continuous nursing care on inhalation therapy and domiciliary oxygen therapy compliance in patients with bronchial asthma-chronic obstructive pulmonary disease overlap syndrome(ACOS).Methods 71 patients with stable ACOS from August 2016 to January 2018 were selected and randomly divided into the control group(36 patients)and the experimental group(35 patients).The control group received conventional nursing,and the experimental group received conventional nursing plus three months of continuous nursing,including multiple telephone follow-ups to guide patients for inhalation therapy and domiciliary oxygen therapy,sending educational videos through WeChat platform,answering questions,and conducting diet,exercise and condition monitoring and psychological care,etc.After three months,the compliance of inhalation therapy and domiciliary oxygen therapy,as well as quality of life score before and after intervention were compared between the two groups.Results After intervention,the compliance of inhalation therapy and domiciliary oxygen therapy in the experimental group was higher than that in the control group(P<0.05).The St.George's Respiratory Questionnaire(SGRQ)scores of the experimental group was lower than the control group(P<0.05).Conclusion Continuous nursing care can effectively improve the compliance of inhalation therapy and domiciliary oxygen therapy in patients with bronchial asthma-chronic obstructive pulmonary disease overlap syndrome and improve their quality of life.
