腹腔镜直肠癌根治术盆腔自主神经保护技术的临床应用及改良

Clinical application and improvement of laparoscopic radical rectectomy for rectal cancer and pelvic autonomic nerve protection technique

目的探讨腹腔镜直肠癌根治术盆腔自主神经保护技术的临床应用及改良。方法依据是否采用神经保护技术选取2014年2月~2017年6月采用腹腔镜手术治疗但未采用神经保护技术的50例直肠癌患者为对照组,2017年7月~2019年3月收治直肠癌并行腹腔镜直肠全系膜切除术(TME)及盆底神经保护的50例患者为观察组,观察两组患者的术后6周的排尿功能与性功能情况,并观察两组患者的手术时间与住院时间等。结果观察组术后6周排尿功能与性功能显著改善,并且住院时间明显较短,但观察组的手术时间明显长于对照组,差异有统计学意义(P<0.05)。结论腹腔镜直肠癌根治术联合盆腔自主神经保护技术具有显著的临床效果。

Objective To explore the clinical application and improvement of laparoscopic radical rectectomy for rectal cancer and pelvic autonomic nerve protection technique.Methods According to whether nerve protection technique was used or not,50 patients with rectal cancer who underwent laparoscopic surgery without adoption of nerve protection technique from February 2014 to June 2017 were selected as the control group.50 patients with rectal cancer who underwent laparoscopictotal mesorectal excision(TME)combined with pelvic floor nerve protection from July 2017 to March 2019 were selected as the observation group.The urination function and sexual function of the patients in the two groups were observed 6 weeks after the operation,and the operation time and hospital stay of the patients in the two groups were also observed.Results The urination function and sexual function were improved significantly in the observation group at 6 weeks after operation.In addition,the hospital stay of the observation groupwas significantly shorter.The operation time in the observation group was longer than that in the control group,and the difference was significant(P<0.05).Conclusion Laparoscopic radical rectectomy for rectal cancer combined with pelvic autonomic nerve protection technique has significant clinical curative effects.
