复方氟米松软膏联合消银颗粒治疗寻常型银屑病效果及其对外周血嗜酸性粒细胞P物质和NK1R的影响

Clinical efficacy of compound fluoromepine ointment combined with Xiaoyin Granules in patients with psoriasis vulgari and its effects on peripheral blood eosinophils SP and NK1R

目的探究复方氟米松软膏联合消银颗粒对寻常型银屑病患者的疗效及对外周血嗜酸性粒细胞P物质及神经激肽/速激肽受体1(neurokinin/tacheptide receptor 1,NK1R)的影响。方法选取银屑病患者120例,按照随机数字表法分为研究组和对照组各60例。研究组给予复方氟米松软膏联合消银颗粒治疗,对照组给予复方氟米松软膏治疗。观察2组临床疗效,比较2组治疗前后瘙痒评分、银屑病面积和严重程度指数(Psoriasis Area and Severity Index,PASI)、皮肤病生活质量指数(Dermatology Life Quality Index,DLQI)、复发率、血清白细胞介素8(interleukin-8,IL-8)和肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)水平、外周血嗜酸性粒细胞P物质和NK1R水平及药物不良反应。结果治疗后研究组临床疗效优于对照组,治疗有效率高于对照组(P<0.05)。治疗后2组瘙痒评分、PASI评分、DLQI评分均明显低于治疗前,研究组瘙痒评分、PASI评分、DLQI评分明显低于对照组(P<0.05)。研究组复发率显著低于对照组(P<0.05)。2组IL-8和TNF-α水平明显低于治疗前,研究组血清IL-8和TNF-α水平明显低于对照组(P<0.05)。2组外周血嗜酸性粒细胞P物质及NK1R比例明显低于治疗前,研究组外周血嗜酸性粒细胞P物质及NK1R比例明显低于对照组(P<0.05)。2组不良反应发生率差异无统计学意义(P>0.05)。结论复方氟米松软膏联合消银颗粒能够有效治疗寻常型银屑病,改善患者临床症状,降低炎症因子IL-8和TNF-α水平,下调嗜酸性粒细胞P物质和NK1R比例,减少皮肤瘙痒和炎症反应,安全有效,值得临床进一步研究并推广。

Objective To explore the clinical efficacy of compound fluoromepine ointment combined with Xiaoyin granules in patients with psoriasis vulgari and its effects on peripheral blood eosinophils P substance and neurokinin/tacheptide receptor 1(NK1R).Methods One hundred and twenty cases of psoriasis vulgari patients were selected as the observation subject,and the patients were divided into the control group and the study group according to the random number table,60 cases in each group.The control group was treated with compound flumetasone ointment,while the study group was treated with the Xiaoyin granules on the basis of the control group.The clinical efficacy,the scores of Psoriasis Area and Severity Index(PASI)and NK1R,recurrence rate,the serum levels of interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α),the levels of P substance and NK1R in eosinophils and the adverse reactions between two groups were compared and analyzed.Results After the treatment,the clinical efficacy in the study group was 91.67%,which was significant higher than that in the control group 76.67%(P<0.05).The recurrence rate in the study group was 16.67%,which was lower than that in the control group 35.00%(P<0.05).The pruritus score,PASI score and Dermatology Life Quality Index(DLQI)score in the two groups were significantly decreased,and the pruritus score,PASI score and DLQI score in the study group were significantly lower than those in the control group.The levels of IL-8 and TNF-α in the two groups were significantly decreased,and the serum levels of IL-8 and TNF-α in the study group were significantly lower than those in the control group(P<0.05).The proportion of eosinophil P substance and NK1R in peripheral blood in the two groups was significantly decreased,and the proportion of eosinophil P substance and NK1R in the study group was significantly lower than that in the control group(P<0.05).In addition,there was no significant difference in the incidence of adverse reactions between two groups(P<0.05).Conclusion The compound flumetasone ointment combined with Xiaoyin Granules in the treatment of psoriasis vulgari can effectively improve the clinical efficacy,decrease the levels of IL-8 and TNF-α,down-regulate the levels of P substance and NK1R in eosinophils,it is safe and worth further research and promotion.
