摘要
目的 探讨神经内镜辅助技术在显微血管减压术中的意义。方法 在神经内镜辅助下实施显微血管减压术治疗颅神经疾病52例。在显微镜下显露神经及责任血管,垫入隔片前后均置入神经内镜观察以明确责任血管、压迫部位及隔片位置。结果 52例中有8例应用神经内镜探查时发现遗漏并存的责任血管,12例内镜探查后调整隔片位置。术后1周治愈42例,好转10例。随访时间3~42个月,平均24个月,仅1例三叉神经痛复发。结论 神经内镜辅助显微血管减压术能避免遗漏责任血管、协助调整隔片位置,提高手术有效性,降低术后复发率。
Objective To explore the significance of neuroendoscope-assisted technique in microvascular decompression.Methods 52 cases of cranial nerve disease were treated by microvascular decompression assisted by neuroendoscope.The nerve and the responsible vessel were exposed under the microscope,and neuroendoscopy was inserted before and after the septum insertion to determine the responsible vessel,the compression site and position of the septum.Results Among the 52 cases,8 cases were found to have missed the co-existed responsible vessel during endoscopic exploration,and 12 cases adjusted the septum position after endoscopic exploration.42 cases were cured and 10 cases were improved one week after surgery.The follow-up time was 3~42 months,with an average of 24 months.Only 1 case relapsed trigeminal neuralgia.Conclusion Microvascular decompression assisted by neuroendoscope can avoid omissions of responsible vessels,assist in adjusting position of the septum,improve the effectiveness of surgery,and reduce postoperative recurrence rate.
