腕桡侧入路掌侧锁定钢板内固定治疗桡骨远端骨折的效果

Effect of Carpal Radial Approach with Palmar Locking Plate Internal Fixation on Distal Radius Fractures

目的探讨腕桡侧入路掌侧锁定钢板内固定对桡骨远端骨折患者术中出血量及腕关节功能的影响。方法选取2016年8月~2019年8月我院收治的80例桡骨远端骨折患者,随机分为对照组及观察组,各40例,对照组予以腕桡侧入路掌侧普通钢板内固定治疗,观察组予以腕桡侧入路掌侧锁定钢板内固定治疗,比较两组手术时间、术中出血量、住院时间、骨折愈合时间及术前、术后3个月疼痛程度和腕关节功能。结果两组手术时间、术中出血量及住院时间比较,差异无统计学意义(P>0.05);观察组骨折愈合时间短于对照组,差异有统计学意义(P<0.05);两组术前VAS评分、PRWE评分以及Gartland-Werley评分比较,差异无统计学意义(P>0.05);术后3个月两组VAS评分、Gartland-Werley评分下降,PRWE评分上升,且观察组改善优于对照组,差异有统计学意义(P<0.05)。结论腕桡侧入路掌侧锁定钢板内固定治疗桡骨远端骨折效果确切,有助于缩短骨折延迟愈合时间、减轻患者疼痛,改善患者腕关节功能。

Objective To investigate the effect of radial fixation with volar locking plate internal fixation on carpal radial approach on intraoperative blood loss and wrist function in patients with distal radius fractures.Methods A total of 80 patients with distal radius fractures treated in our hospital from August 2016 to August 2019 were randomly divided into the control group and the observation group,each with 40 cases.Treatment,the observation group was treated with the radial approach of the palmar locking plate internal fixation,and the operation time,intraoperative blood loss,hospital stay,fracture healing time,and the pain and wrist function before and after 3 months were compared between the two groups.Results There was no significant difference in the operation time,intraoperative blood loss,and hospital stay between the two groups(P>0.05).The fracture healing time in the observation group was shorter than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in VAS score,PRWE score,and Gartland-Werley score(P>0.05).After 3 months,the VAS score and Gartland-Werley score of the two groups decreased,and the PRWE score increased,and the observation group improved better than the control group,the difference was statistically significant(P<0.05).Conclusion The radial-carpal approach of the palmar locking plate internal fixation is effective in treating distal radius fractures,which is helpful to shorten the delayed healing time of fractures,reduce pain,and improve wrist function.
