新斯的明双侧足三里穴位注射联合芒硝外敷对妇科腹腔镜术后恶心呕吐的影响

The effect of neostigmine injection at Zusanli and mirabilite external application on nausea and vomiting after gynecological laparoscopic surgery

目的探讨双侧足三里新斯的明穴位注射联合芒硝外敷对妇科腹腔镜术后患者恶心呕吐的影响。方法选择2014年9月~2016年3月在我院行妇科腹腔镜手术的198例患者为研究对象,按随机数字表法分为4组,即常规护理组(对照组)48例,双侧足三里新斯的明穴位注射组(注射组)50例,芒硝外敷组(外敷组)50例,双侧足三里新斯的明穴位注射联合芒硝外敷组(联合组)50例,观察比较四组患者腹腔镜术后不同时间点(1 h、6 h、12 h、24 h)腹胀程度及恶心呕吐发生率。结果注射组、外敷组及联合组3组干预组患者术后腹胀程度均低于对照组,差异具有统计学意义(均P<0.05);而且联合组的效果优于注射组、外敷组,差异具有统计学意义(均P<0.05)。3组干预组患者术后6 h的恶心呕吐发生率均低于对照组,差异具有统计学意义(P<0.05)。结论双侧足三里新斯的明穴位注射联合芒硝外敷能降低妇科腹腔镜术后患者腹胀程度,有效预防恶心呕吐现象,值得临床推广应用。

Objective To explore the effect of neostigmine injection at Zusanli and mirabilite external application on nausea and vomiting after gynecological laparoscopic surgery. Methods 198 patients who underwent gynecologic laparoscopic surgery in our hospital from September 2014 to March 2016 were selected as the rearch objects.they were randomly divided into four groupsby digital method,48 in the control group and 50 in each of the other three groups,namely,the control group,the injection group(bilateral Zusanli Xinsi Mingding acupoints group),the external application group(mirabilite external application group),the combined treatment group(both neostigmine injection into bilateral Zusanli Xinsi Mingding and mirabilite external application group).The abdominal distension degree and the incidence rate of nausea and vomiting at differentce time points(1 h,6 h,12 h,24 h)after laparoscopic surgery were compared. Results The postoperative extent of abdominal distension and incidence of nausea and vomiting were lower in the three intervention groups than in the observation group,with statistically significant differences(P<0.05 for each). Conclusion Neostigmine injection into bilateral Zusanli acupoints combined with topical Glauber’s salt is effective for preventing nausea and vomiting after gynecologic laparoscopic surgery,supporting its further clinical application.
