探讨颅骨缺损修补物外露用中厚皮片植皮辅助治疗的临床疗效

Therapeutic Effects of Skin Grafting with Intermediate Thickness Free Skin Graft in Treatment of Titanium Mesh Outcrop After Skull Defect Neoplasty

目的分析颅骨缺损修补物外露用中厚皮片植皮辅助治疗的临床疗效。方法选择2014年3月-2019年3月周口市中医院收治的颅骨缺损修补物外露患者64例实施分组研究,以随机数表法分为两组,对照组用皮肤减张缝合处理,观察组用头皮转移瓣辅之以中厚皮片植皮处理。两组均随访3个月以上,记录手术时间、手术出血量、愈合率、创面愈合时间,并调查患者对塑型满意率与随访3个月修补物再次外露率,实施组间对比。结果观察组手术时间短、出血量少、愈合率高、创面愈合时间短,且塑型满意率高、随访3个月修补物再次外露率低,与对照组对比差异均有统计学意义(P<0.05)。结论颅骨缺损修补物外露采取中厚皮片植皮辅助治疗,相比皮肤减张缝合处理,可缩短手术时间与创面愈合时间,减少手术出血和随访期间修补物再次外露,提高愈合率与塑型满意率,值得临床应用。

Objective To analyze the clinical effect of skin grafting with skin grafting with intermediate thickness free skin graft in treatment of titanium mesh outcrop after skull defect neoplasty.Methods A total of 64 patients who underwent cranial defect neoplasty in zhoukou hospital of traditional Chinese medicine from March 2014 to March 2019 were divided into two groups by random number table method.The patients in control group were treated by skin tension-relieving suture,however,the patients in research group were treated by scalp transfer valve combined with skin grafting with intermediate thickness free skin graft.Both groups were followed up for more than 3 months,and the operation time,the amount of surgical bleeding,the healing rate and the wound healing time were recorded.The satisfaction rate of the patients on the model and the incidence rate of titanium mesh outcrop were investigated and compared between two groups.Results The observation group had shorter operation time,lower blood loss,highrt healing rate,shorter wound healing time,higher satisfaction rate of molding,and lower incidence rate of titanium mesh outcrop during follow-up of 3 months,all of which were statistically significant compared with the control group(P<0.05).Conclusion Compared with the treatment of skin tension-relieving suture,the skin grafting with intermediate thickness free skin graft in treatment of titanium mesh outcrop after skull defect neoplasty can not only shorten the surgery time and wound healing time and reduce the surgical bleeding and the incidence rate of titanium mesh outcrop during follow-up,but also improve the rate of healing and mold satisfaction,which is worthy of clinical application.
