超声引导经皮穿刺子宫肌瘤微波消融术的并发症及防治

Prevention and treatment of complications of ultrasound guided percutaneous microwave ablation for uterine fibroid

目的探讨超声引导经皮穿刺子宫肌瘤微波消融术并发症的发生原因及防治措施。方法对2016年4月至2019年4月253例子宫肌瘤患者进行718点微波消融治疗,回顾性分析并发症发生的原因,总结防治策略。结果与单点消融患者比较,205例多点消融患者术后发热、疼痛、腰部酸痛、阴道排液、恶心呕吐的发生率及程度显著增加,并且随消融点数的增加而上升。围手术期子宫肌瘤患者具有相应临床特征,多点微波消融术后并发症发生率高,程度较重,但根据其临床特点加强超声引导经皮穿刺微波消融的围手术期护理,并发症均可逆转。结论了解超声引导经皮穿刺子宫肌瘤微波消融手术并发症及其影响因素,以采取相应措施预防其发生。

Objective To investigate the complications caused by ultrasound. guided percutaneous microwave ablation(MWA) for uterine fibroid and analyze the prevention approach.Methods A total of 253 uterine fibroid patients(718 MWA treatments) in our hospital were enrolled in this study between April 2016 and April 2019. This retrospective study analyzed the reasons and the strategies for preventing the complications. Results Compared with single-point ablation,the incidence and degree of postoperative fever,pain,lumbago, nausea and vaginal discharge in 205 patients with multipoint ablation were significantly higher,and the incidence and degree of postoperative complications increased with the number of ablation points. Perioperative period nursing care of patients with ultrasound-guided percutaneous MWA for uterine fibroid had its particularity and was closely related to their clinical characteristics.The incidence of complications after multi-point MWA for uterine fibroid is high and the degree of complications is serious.However,strengthening perioperative nursing according to its characteristics can be reversed. Conclusions Understanding the complications and influence factors of ultrasound-guided percutaneous MWA for uterine fibroid can help to take effective measures to prevent the occurrence of complications.
