当归补血汤对结直肠癌围手术期患者肠道菌群及机体免疫功能的影响

Effects of Danggui Buxue Decoction on Intestinal Flora and Body Immune Function of the Patients with Colorectal Cancer over Perioperative Period

目的:观察当归补血汤对结直肠癌围手术期患者肠道菌群及机体免疫功能的影响。方法:将结直肠癌围手术期患者170例随机分为2组各85例,对照组术前3天及术后3天分别应用甲硝唑、头孢哌酮舒巴坦,试验组术前5天及术后7天应用当归补血汤。观察2组患者术前、术后7天免疫反应指标及术后7天肠道菌群分布情况,同时观察2组患者术前及术后1、2、4月肛肠动力学情况。结果:术后7天2组患者肠道菌群分布均为厚壁菌门、拟杆菌门、变形菌门、梭杆菌门、放线菌门及其他菌门,除梭杆菌门试验组低于对照组外,其他各菌门数量试验组均高于对照组,差异具有统计学意义(P<0.001);术后7天2组患者免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、免疫球蛋白A(IgA)及CD4+/CD8+均上升,但试验组高于对照组,差异具有统计学意义(P<0.001)。试验组患者术后1、2、4月肛管高压带、肛管静息压、肛管最大收缩压低于对照组(P<0.05)。结论:当归补血汤可有效提高结直肠癌围术期患者机体免疫功能,调整肠道菌群分布,改善肛肠动力学。

Objective:To observe the effects of Danggui Buxue decoction on intestinal flora and body immunologic function in colorectal cancer patients during perioperative period.Methods:All 170 patients were randomized into two groups,85 cases each group,the control group used metronidazole and ceftrione sulbactam three days before surgery and three days after the surgery respectively,the experiment group took Danggui Buxue decoction five days before surgery and seven days after the surgery.To observe immune response index before the surgery,seven days after the surgery,the distribution of intestinal flora seven days after the surgery in both groups,simultaneously,to observe anorectal dynamics before the surgery,one month,two and four months after the surgery in both groups.Results:Intestinal flora in both groups seven days after the surgery were Phylum Firmicutes,Bacteroides,Proteobacteria,Fusobacteria,Actinobacteria and other fungus,the experiment group was higher than the control group in others except fusobacteria(P<0.001).IgG,IgM,IgA and CD4+/CD8+in both groups seven days after surgery lifted,and the experiment group was higher than the control group,and the difference could be found(P<0.001).The experiment group was lower than the control group in anal high-pressure band,anal resting pressure and maximum systolic pressure of anal canal one month,two months and four months after the surgery(P<0.05).Conclusion:Danggui Buxue decoction could effectively raise body immunologic function of the patients suffering colorectal cancer in perioperative period,regulate the distribution of intestinal flora and improve anorectal kinetics.
