摘要
目的探讨血清尿酸、25-羟维生素D水平与前段非动脉炎性缺血性视神经病变(NAION)的相关性。方法选取103例NAION患者为研究对象,根据病程分为病程1组、病程2组、病程3组和病程4组;根据患者视盘不同水肿程度分为视网膜消退组、轻度水肿组和重度水肿组,并以同期100例健康体检者作为对照,检测所有受试者血清尿酸和25-羟维生素D水平,Pearson相关性分析血清尿酸、25-羟维生素D与NAION患者病程和视盘水肿程度相关性,Logistic分析NAION的影响因素。结果与对照组相比,NAION患者血清尿酸表达水平显著升高(P<0.05),25-羟维生素D表达水平显著降低(P<0.05)。随着NAION患者病程的增加,血清尿酸呈上升趋势,25-羟维生素D呈下降趋势,且不同病程组间两两比较差异均显著(P<0.05)。随着NAION患者视盘水肿程度的增加,血清尿酸也呈上升趋势,25-羟维生素D表达水平呈下降趋势,且不同水肿程度组间两两比较差异均显著(P<0.05)。Pearson相关性分析结果显示,血清尿酸与患者病程和视盘水肿程度均呈正相关(P<0.05),25-羟维生素D与患者病程和视盘水肿程度均呈负相关(P<0.05)。Logistic回归分析结果显示,尿酸和25-羟维生素D均是NAION的独立影响因素(P<0.05)。结论 NAION患者血清尿酸水平升高,25-羟维生素D水平降低,可能参与NAION疾病的发生和发展。
Objective To investigate the correlations between serum uric acid and 25-hydroxyvitamin D levels with nonarteritic anterior ischemic optic neuropathy(NAION).Methods One hundred and three patients with NAION were selected as the study subjects, who were further divided into course 1 group, course 2 group, course 3 group, and course 4 group, according to the course of disease. The study subjects were also divided into retinal regression group, mild edema group and severe edema group, according to the degree of optic disc edema. Another 100 healthy people in the same period were enrolled as the control group. Serum uric acid and 25-hydroxyvitamin D levels were measured in all subjects. Pearson correlation analysis was used to analyze the correlations between serum uric acid and 25-hydroxyvitamin D levels with the course of NAION and degree of optic disc edema. Logistic regression was used to analyze the risk factors for NAION. Results Compared with the control group, the expression of serum uric acid in NAION patients increased significantly(P<0.05), while the expression of 25-hydroxyvitamin D decreased significantly(P<0.05). With longer course of NAION, serum uric acid level increased and 25-hydroxyvitamin D level decreased, and there were significant differences among groups with different course of NAION(P<0.05). With more severe optic disc edema in NAION patients, serum uric acid level showed an increasing trend, and the 25-hydroxyvitamin D slevel howed a decreasing trend, and there were significant differences among groups with different degrees of edema(P<0.05). Pearson correlation analysis showed that serum uric acid level was positively correlated with the course of disease and degree of optic disc edema(P<0.05), and 25-hydroxyvitamin D level was negatively correlated with the course of disease and degree of optic disc edema(P<0.05). Logistic regression analysis showed that the uric acid level and 25-hydroxyvitamin D level were independent influencing factors of NAION(P<0.05). Conclusions The serum uric acid level in patients with NAION increases and 25-hydroxyvitamin D level decreases, both factors may be involved in the development and progression of NAION.
