院前指数评价急诊创伤住院患者的价值探究

Exploration on the value of pre-hospitalindex in evaluating emergency trauma inpatients

目的探究院前指数评价急诊创伤住院患者的价值。方法选择2018年7~12月我院收治的200例急诊创伤住院患者为研究对象,本实验要利用我院自行设计的院前急救记录表,全面记录好病患基本详情。开展PHI评分,同时和ISS评分相比,探究结果。结果在200例住院患者之中治愈者共计184例,占92%。死亡6例,占3%。残疾10例,占5%。从患者死亡原因来看。颅脑损伤致死4例,胸部损伤致死1例,肝破裂致死1例。从致残方面来看,肢体离断1例,周围神经损伤致残1例,脊髓损伤3例,颅脑损伤致残5例。治愈者院前评分均值为(2.59±1.40)分,院内评分为(2.50±0.29)分。死亡患者院前评分为(16.7±2.45)分,院内评分为(15.81±2.52)分。残疾患者院前评分为(7.59±2.13)分,院内评分为(6.02±1.62)分。结论对于急诊创伤住院患者来讲开展院前指数评价(PHI),有着相当重要的现实意义。其能够良好的评估患者伤情,方便为患者进行抢救,特异性强,有效性高,因此值得进一步在临床中推广应用。

Objective To explore the value of pre-hospital index in evaluating emergency trauma inpatients. Methods Two hundred emergency trauma inpatients who were admitted and treated in our hospital from July to December 2018 were selected as research objects. This experiment used the pre-hospital emergency record form designed by our hospital to comprehensively record the basic details of the patients. The PHI score was carried out, and the ISS score was compared to explore the results. Results Among the 200 inpatients, 184 were cured, accounting for 92%. 6 cases died, accounting for 3%. There were 10 cases of disability, accounting for 5%. From the perspective of causes of death, four cases died of craniocerebral injury, one case died of thoracic injury and one case died of liver rupture. In terms of disability, there were 1 case of limb amputation, 1 case of peripheral nerve injury, 3 cases of spinal cord injury and 5 cases of craniocerebral injury. The mean pre-hospital score of cured patients was(2.59±1.40) score and in-hospital score was(2.50±0.29) score. The pre-hospital and in-hospital scores of the dead patients were respectively(16.7±2.45) score and(15.81±2.52) score. The pre-hospital and in-hospital scores of disabled patients were(7.59±2.13) score and(6.02±1.62) score. Conclusion Pre-hospital index(PHI) evaluation is of great practical significance for emergency trauma inpatients. It can evaluate the patient’s condition well and it is convenient to rescue the patients. It has strong specificity and high effectiveness. Therefore, it is worth further promoting and applying in clinical practice.
