患者自主疼痛评估在全膝关节置换术围术期疼痛管理中的应用价值分析

Analysis on application values of autonomous pain assessment of patients in perioperative pain management of total knee arthroplasty

目的探究患者自主疼痛评估在全膝关节置换术围术期疼痛管理中的应用价值。方法将本院接受全膝关节置换术的66例患者作为研究对象,其治疗时间为2014年9月~2018年9月。将其随机分为参照组与研究组,每组33例,参照组由护士进行疼痛评估,研究组则通过患者自主疼痛评估,根据两组相应的评估结果进行处理。观察记录两组评估效果与康复效果。结果研究组患者术后平均每日疼痛评分低于参照组,解救次数低于参照组,术后疼痛总评分低于参照组,差异有统计学意义(P <0.05);术前两组患者自我效能评分比较差异无统计学意义(P> 0.05),术后3、9、15d自我效能评分高于参照组,差异有统计学意义(P <0.05)。结论通过对患者采用自主疼痛评估可有效促进围术期疼痛管理,缓解疼痛情况,有利于患者全膝关节置换术治疗的康复与预后。

Objective To explore the application value of autonomous pain assessment of patients in pain management in perioperative pain management of total knee arthroplasty. Methods 66 patients who underwent total knee arthroplasty in our hospital from September 2014 to September 2018 were selected as research objects.They were randomly divided into the reference group and the research group,with 33 cases in each group.The reference group was given pain assessment by nurses while the research group was given autonomous pain assessment of patients.They were given corresponding measures according to evaluation results.The evaluation effect and rehabilitation effect of the two groups were observed and recorded. Results The average daily pain score of the research group was lower than that of the reference group,the frequency of rescues was lower than that of the reference group and the total postoperative pain score was lower than that of the reference group.The difference was statistically significant(P < 0.05).There was no difference in self-efficacy score between the two groups before operation(P > 0.05).The self-efficacy score was higher than that of the reference group on the 3 rd,9 th and 15 th days after operation.The difference was statistically significant(P < 0.05). Conclusion Autonomous pain assessment of patients can effectively promote perioperative pain management and relieve pain,which is conducive to the rehabilitation and prognosis of patients undergoing total knee arthroplasty.
